Microperimetry in exudative age-related macular degeneration

Abstract:

Úvod: Mikroperimetrie je vyšetřovací metoda, která umožňuje hodnotit výsledky léčby makulárních nemocí v průběhu času. Naší snahou bylo zhodnotit přínos mikroperimetrie u pacientů s vlhkou formou věkem podmíněné makulární degenerace pomocí bevacizumabu, srovnat měřené veličiny na mikroperimetru (retinální citlivost, absolutní skotom) a vyhodnotit zrakovou ostrost měřenou na Snellenově optotypu před léčbou a měsíc po léčbě. Metody: Do studie bylo zařazeno 21 pacientů (21 očí) s vlhkou formou VPMD, kteří byli měřeni na přístroji OCT/SLO, jehož součástí je mikroperimetrický test. U každého pacienta byla provedena dvě mikroperimetrická měření - den před aplikací léčby a měsíc po aplikaci léčby. Na mikroperimetru byly zaznamenány hodnoty retinální citlivosti a případné absolutní skotomy. Zároveň byla pacientům měřena hodnota zrakové ostrosti s nejlepší možnou korekcí. Výsledky: V souboru 21 očí u 8 mužů a 13 žen ve věku 50 – 86 let se vyhodnocovaly rozdíly (na zvolené hladině významnosti 0,05) mezi pohlavím u veličin – věk, retinální citlivost, absolutní skotom, visus. Rozdíly mezi muži a ženami byly nalezeny u retinální citlivosti a to před léčbou (p = 0,006) i po léčbě (p = 0,013). U věku, zrakové ostrosti ani absolutního skotomu rozdíl mezi pohlavím nebyl zjištěn. Citlivost sítnice se při fixaci pacienta do 4 stupňů zvýšila ve skupině mužů ze 14,49 dB ± 2,44 dB na 15,65 dB ± 2,61 dB (p = 0,021), ve skupině žen z 10,47 dB ± 3,11 dB na 12,05 dB ± 3,10 dB (p = 0,030). Zraková ostrost v celém souboru (muži + ženy) vzrostla z 0,48 ± 0,17 na 0,60 ± 0,18 (p = 0,000), zlepšila se o 12%. Statisticky významný je i výsledek léčby na absolutní skotom (p = 0,005). Závěr: Mikroperimetrie představuje vhodnou metodu ke zhodnocení funkce makulární oblasti sítnice. Studie ukazuje, že léčba vlhké VPMD intravitreální injekcí bevacizumabem prokazuje signifikantní zvýšení retinální citlivosti, snížení absolutního skotomu a zlepšení zrakové ostrosti po měsíci léčby.

Abstract:

Introduction: Microperimetry is a diagnostic technique that allows to evaluate the results of treatment of macular disease over time. Our aim was to evaluate the benefit of microperimetry in patiens with exudative age-related macular degeneration using bevacizumab, to compare the measured values using the microperimetry ( retinal sensitivity, absolute scotoma) and to evaluate the visual acuity measured on Snellen chart, before treatment and one month after treatment. Methods: The study included 21 patients (21 eyes) with exudative AMD who were measured on a OCT/SLO, which includes microperimetric test. Each patient underwent two microperimetric measurements - one day prior to treatment, and one month after application of the treatment. The microperimetr recorded values of the retinal sensitivity and the absolute scotoma if occured. The value of visual acuity with the best possible correction were measured in patients at the same time. Results: In the group of 21 eyes in 8 men and 13 women aged 50-86 years were evaluated differences ( the significance level 0.05 was chosen) between genders variables - age, retinal sensitivity, absolute scotoma, visus. The differences between men and women were found prior to the treatment (p = 0.006) and after the treatment (p = 0.013) in retinal sensitivity. The differences were not found between gender according to age, visual acuity or absolute scotoma. The sensitivity of the retina during fixation of the patient into 4 levels increased in the group of men from 14.49 dB ± 2.44 dB to 15.65 dB ± 2.61 dB (p = 0.021), in the group of women from 10.47 dB ± 3.11 dB to 12,05 dB ± 3.10 dB (p = 0.030). The visual acuity in the entire group (men + women) increased from 0.48 ± 0.17 to 0.60 ± 0.18 (p = 0.000), improved by 12 %. The result of the treatment of absolute scotoma is statistically significant too (p = 0.005). Conclusion: Microperimetry is a suitable method to evaluate the function of the macular region of the retina. The study shows that the treatment of exudative AMD intravitreal injection of bevacizumab demonstrated a significant increase in retinal sensitivity, reduction in the absolute scotoma and the improvement of the visual acuity after one month of treatment.
