Úloha microRNA v biologii a terapeutické rezistenci u B-buněčných malignit

The role of microRNAs in the biology and therapeutic resistance of B-cell malignancies

Abstract:

Jedním z hlavních témat nádorové biologie je pochopení regulačních mechanismů zprostředkovaných skrz microRNA (miRNA) a jejich role v patogenezi onkologických onemocnění a to od propojení deregulace klastru miR-15/16 s chronickou lymfocytární leukemií (CLL) v roce 20021. Předkládaná práce prezentovaná jako komentovaný soubor prací se zabývala zejména studiem miRNA u CLL a částečně u folikulárního lymfomu (FL), neboť obě onemocnění představují časté indolentní B buněčné malignity v západních zemích s heterogenním průběhem a ne zcela objasněnou patogenezí. Během doktorského studia bylo třeba zavést řadu metodik pro analýzu miRNA, jichž bylo využito jak v komentovaných článcích, tak přiloženém patentu. Role miR-34a je komentována zejména s ohledem na dráhu odpovídající na poškození DNA, která je stěžejní v odpovědi na standardní terapii. Nově byl také popsán regulační mechanismus zahrnující p53/miR-34a/FOXP1, který propojuje regulaci v odpověď na poškození DNA s B-buněčnou signalizací (BCR). Relevance miR-34a v patogenezi CLL byla také zdůrazněna díky možnosti jejího využití jako biomarkeru predikujícího odpověď na terapii FCR (fludarabin, cyklofosfamid, rituximab) a markeru kratší doby bez progrese onemocnění v léčené skupině pacientů a markeru celkové doby přežití a to nezávisle na statutu p53. Byla současně zavedena metoda absolutní kvantifikace kopií miR-34a ve vzorku a spolu s ní definovány přesné hraniční počty molekul identifikující pacienty s potenciálně agresivním onemocněním, nebo pacienty s abnormalitou v p53 (delece a/nebo mutace). Role FOXP1 byla dále studována ve spojitosti s miR-150 a folikulárním lymfomem s důrazem na jeho transformaci, neboť miR-150 je během této transformace uniformě down-modulována. Dráha MYC/miR-150/FOXP1 byla dále popsána jako dráha zodpovědná za agresivitu a transformaci folikulárního lymfomu. Celkově předkládaná data přispěla k porozumění úloze miRNA v patogenezi CLL a FL a zdůraznila význam miRNA jakožto prediktivních a prognostických markerů využitelných v hematoonkologii.

Abstract:

One of the main topics in cancer biology is the understanding of microRNAs (miRNAs) regulatory mechanisms and their role in pathogenesis of oncological diseases since the time the connection was established in 2002 for miR-15/16 cluster and chronic lymphocytic leukemia (CLL)1. The offered thesis presented as a commented set of publications focused on the study of miRNAs mainly in CLL and partly in follicular lymphoma (FL) as both of these diseases represent common indolent B-cell malignancies in Western countries with heterogeneous outcome and not fully explained pathogenesis. During the doctoral study several methodologies for miRNAs analyses were established and utilized in the commented articles and patent. The role of miR-34a in the context of induced DNA damage response (DDR) crucial for the response to the standard therapy was evaluated and novel regulatory circuit p53/miR-34a/FOXP1 was described connecting the DDR to B-cell receptor signaling pathway (BCR). The relevance of miR-34a in CLL pathogenesis was also highlighted by its ability to serve as a biomarker for worse response or shorter progression free survival in CLL patients treated with FCR regimen (fludarabine, cyclophosphamide, rituximab), and a biomarker of shorter overall survival, irrespective of p53 status. The method for absolute quantification of miR-34a copies was established, and prognostic cut-offs were set to identify patients with potentially risky disease or patients with p53 abnormalities (patented as a biomarker in CLL and FL). The FOXP1 function was also studied in the connection to miR-150 in FL with the emphasis on FL transformation as miR-150 was uniformly down-modulated after the transformation. The axis MYC/miR-150/FOXP1 was further demonstrated to be a determinant of FL aggressiveness and transformation. All together the presented data significantly contributed to the understanding of miRNAs role in CLL and FL pathogenesis and underscored miRNAs’ ability to serve as prognostic and predictive markers in hematological malignancies.
