Usage of the prone position as a part of modern ARDS treatment

Abstract:

Diplomová práce je zaměřena na terapii pacientů s akutním respiračním selháváním, a to konkrétně na využití pronační polohy jako součásti moderní léčby ARDS. Teoretická část se zabývá ARDS a mimo jeho vlastní popis je zaměřena na farmakologické a nefarmakologické léčebné možnosti tohoto onemocnění. Jako jedna z nefarmakologických metod terapie je zařazena pronační poloha, která je dále předmětem výzkumné části. Kromě indikací, kontraindikací a výskytu komplikací, které jsou s pronací spojené, je zde popsaná specifická ošetřovatelská péče o pacienty v pronační poloze. V empirické části je uveden systematický přehled doporučení a názorů zahraničních zdrojů na provádění a prospěšnost pronační polohy, který byl vypracován dle předem stanovených kritérií. Výsledky vyhledávání relevantních zdrojů jsou pak dále diskutovány s názory na pronační polohu anesteziologicko-resuscitačních klinik vybraných nemocnic, které byly získané krátkou anketou.

Abstract:

The thesis is focused on the therapy of patients with acute respiratory distress syndrome, specifically the use of prone position as a part of modern treatment of ARDS. The theoretical part is concerned with ARDS and apart from its description it presents pharmacological and non-pharmacological ways of treatment. One of the non-pharmacological methods of therapy is the prone position which is the subject of the research part of the thesis. In addition to indications, contraindications and complication occurence connected to proning, the specific nursing care for patients positioned in this way is described. The empirical part contains a systematical survey of recommendations and experience of application and benefits of the prone ventilation taken from foreign sources. The survey was elaborated according to the pre-set criteria. The theoretical research results are compared and discussed further with experience of prone position in ICU of the chosen hospitals obtained via a short questionnaire.
