Hepatitis E virus and its zoonotic potential

Abstract:

Akutní hepatitida E patřila dlouho k opomíjeným onemocněním, v současnosti se však zájem o toto onemocnění rapidně zvýšil. Je charakterizována jako sebelimitující onemocnění, jehož průběh se může závažností lišit od subklinického až po fulminantní. Většinou však probíhá bezpříznakově, u imunosuprimovaných pacientů může přecházet do chronicity. Jejím původcem je virus hepatitidy E (HEV). Jedná se o neobalený RNA virus, který je v současnosti řazen do čeledi Hepeviridae. Jsou známy 4 genotypy HEV, které způsobují onemocnění u člověka. Jednotlivé genotypy se vzájemně liší virulencí, zeměpisnou distribucí i možnými způsoby přenosu. Genotypy HEV-1 a HEV-2 jsou typickými antroponózami, šíří se fekálně-orální cestou a jsou zodpovědné za epidemie v rozvojových zemích. Genotypy HEV-3 a HEV-4 jsou zoonózami a jsou původci autochtonních nákaz v zemích vyspělých. Tyto genotypy a jiné viry blízce příbuzné HEV byly popsány u mnoha zvířecích druhů. Právě zoonotický potenciál HEV vyvolal zájem o toto onemocnění i o jeho původce a jeho bližší výzkum, který se v současnosti zaměřuje na přesnější charakterizaci jednotlivých genotypů a virů podobných HEV s cílem zlepšit diagnostiku a prevenci.

Abstract:

Acute hepatitis E has been underestimated for a long time, but the interest in this disease increased in last years. It is described as self-limiting disease, its progression varies from subclinical to fulminant. It is usually asymptomatic, but in immunocompromised patients, it can develop into chronicity. Hepatitis E virus (HEV) is the causative agent of hepatitis E. It is a non-enveloped RNA virus, and it is classified in the family Hepeviridae. HEV infecting humans is classified into four genotypes. Every genotype differs in its virulence, geographic distribution, and in the ways of transmission. Genotypes HEV-1 and HEV-2 are typical anthroponoses. They are transmitted via fecal-oral route and they are responsible for large outbreaks in developing countries. Genotypes HEV-3 and HEV-4 are zoonotic, and they cause autochthonous infections in developed countries. These genotypes and other HEV-like viruses have been detected in many animal species. The zoonotic potential of HEV has raised interest in this disease, as well as in its causative agent and its closer research. The research is currently focused on more specific characterization of different genotypes and other HEV-like viruses to improve diagnostics and prevention.
