Individual Gestalt therapy for persons diagnosed with schizophrenia

Abstract:

This diploma thesis explores the theme of individual Gestalt therapy for persons diagnosed with schizophrenia. In theoretical part it discusses information about schizophrenia and psychotherapy of schizophrenia, then it introduces Gestalt therapy and finally it presents the review of literature about Gestalt therapy of schizophrenia and psychosis published to this date. Because these texts represented only subjective descriptions of conceptualization of such therapy by a particular author or authors, I decided to grasp this area more rigorously and carry out a qualitative research based on the Grounded theory methodology which brings the resulting model that answers the research question "How do Gestalt therapists work with people diagnosed with schizophrenia?". I conducted interviews with seven Gestalt therapists that had some experience of therapy with these clients. On the basis of analyzing these interviews I created a model with the central category named "Something is missing". In accordance with existing literature, my research results suggest for example a higher level of therapistˈs activity and a higher level of therapistˈs responsibility for a relationship ins activity and a higher level of therapistˈs activity and a higher level of therapistˈs responsibility for a relationship ins responsibility for a relationship in comparison to Gestalt therapy with clients suffering from neurotic difficulties. It also pointed out the emphasis of therapists on the creation of a basic security and on the active support of a contact. This study brings new findings too - it seems that doing therapy with a person diagnosed with schizophrenia may result in frustration of a Gestalt therapist, who on the other hand has a possibility of lowering this frustration by regulating their own demands and expectations from the client and from themselves too. It also seems that Gestalt therapy with these persons may require less horizontal and more authoritative approach from therapist in comparison with the typical Gestalt stance. My thesis results in a detailed description of presented model, discussion and recommendations for future research and practice.

Abstract:

Táto diplomová práca sa zaoberá témou individuálnej Gestalt terapie pre ľudí s diagnostikovanou schizofréniou. V teoretickej časti diskutuje informácie o schizofrénii a psychoterapii schizofrénie, ďalej predstavuje Gestalt terapiu a napokon prináša prehľad doteraz publikovanej literatúry k téme Gestalt terapie schizofrénie a psychóz. Keďže tieto texty predstavovali iba určitý subjektívny popis konceptualizácie takejto terapie daného autora či autorov, rozhodol som sa túto oblasť uchopiť rigoróznejšie a zrealizovať kvalitatívny výskum na základe metodológie zakotvenej teórie, ktorý prináša výsledný model zodpovedajúci výskumnú otázku "Ako Gestalt terapeuti pracujú s ľuďmi s diagnostikovanou schizofréniou?". Viedol som rozhovory so siedmymi Gestalt terapeutmi majúcimi nejakú skúsenosť s terapiou týchto klientov. Na základe ich analýzy som vytvoril model s centrálnou kategóriou nazvanou „Niečo chýba“. Výskumné zistenia v súlade s existujúcou literatúrou naznačili napríklad vyššiu úroveň terapeutovej aktivity a zvýšenú terapeutovu zodpovednosť za vzťah v porovnaní s Gestalt terapiou klientov s neurotickými obtiažami. Ukázal sa tiež dôraz terapeutov na tvorbu bazálneho bezpečia a na aktívnu podporu kontaktu. Táto štúdia prináša i nové zistenia - zdá sa, že terapia s jedincom s diagnostikovanou schizofréniou môže vyvolať frustráciu Gestalt terapeuta, ktorý ale na druhú stranu má možnosť túto frustráciu znižovať reguláciou vlastných nárokov a očakávaní ako od klienta, tak od seba samého. Zdá sa tiež, že Gestalt terapia s týmito jedincami tiež môže vyžadovať od terapeuta menej horizontálny a viac autoritatívny prístup v porovnaní s typickým Gestalt prístupom. Moja diplomová práca prináša podrobný popis modelu, jeho diskusiu a odporúčania ako do praxe, tak aj do ďalšieho výskumného snaženia.
