Inflammatory complication after cardiac surgery

Abstract:

Úvod: Hluboké poruchy hojení podélné střední sternotomie představují závažnou pooperační komplikaci, představující zátěž pro pacienty i kardiochirurgická pracoviště. Cílem naší práce bylo analyzovat hluboké poruchy hojení v Centru kardiovaskulární a transplantační chirurgie Brno (CKTCH) a zjistit možnost ovlivnění incidence hlubokých poruch hojení augmentací primárního uzávěru sterna. Metody: Z dat poskytovaných Národnímu kardiochirurgickému registru České republiky jsme retrospektivně analyzovali rizikové faktory hlubokých poruchy hojení střední sternotomie u pacientů operovaných v CKTCH v letech 2006-2011. Na základě zjištěných výsledků byl vytvořen protokol prospektivní studie, do které byli v letech 2012-2013 randomizováni rizikoví pacienti, u kterých byl primární uzávěr sternotomie proveden augmentovaným uzávěrem. Tuto skupinu pacientů jsme následně statisticky srovnali s pacienty, u kterých byl primární uzávěr sternotomie proveden klasickou drátěnou cerkláží. Výsledky: Retrospektivní analýza – analyzováno bylo 6728 pacientů, u kterých došlo k 154 hlubokým poruchám hojení střední sternotomie (2,29%). Zaznamenali jsme vyšší mortalitu v souboru pacientů s hlubokou poruchou hojení (13% vs. 4%) i prodlouženou délku hospitalizace (41,2 dnů vs. 14,5 dne). Předoperační rizikové faktory byly následující: BMI nad 29,2 kg/m2, předoperační hemodynamická nestabilita a anamnéza plicního onemocnění. Perioperační rizikové faktory: věk nad 65 let, odběr levé vnitřní hrudní tepny (jako bypassu při chirurgické revaskularizaci myokardu), akutní typ operace a doba mimotělního krevního oběhu vyšší než 100 minut. Statisticky významné pooperační rizikové faktory byly: paraparéza, sepse a infekční komplikace, doba pobytu na JIP více než 72 hodin a pooperační psychosyndrom. Prospektivní analýza – u 105 rizikových pacientů (na základě výsledku skórovacího systému a randomizace), u kterých byl použit augmentovaný uzávěr sterna systémem sternálních pruhů, došlo k hlubokému poststernotomickému defektu v 1 případě (0,95%). Ve srovnávací skupině, ve které byla k uzávěru sterna použita klasická drátěná cerkláž, byla incidence hluboké poruchy hojení 4,12% (20 z 486 pacientů). Závěr: V naší studii jsme v první fázi analyzovali rizikové faktory hlubokých poruch hojení v CKTCH Brno. Použití augmentované techniky sternálního uzávěru ve druhé fázi studie vedlo k redukci hlubokých poruch hojení u rizikových pacientů, výsledek však nebyl statististicky významný.

Abstract:

Background: Deep sternal wound defects are a serious postoperative complication, representing a burden for patients and centers of cardiac surgery. The aim of our study was to analyze risk factors of the deep sternal wound infections (DSWI) at the Center of Cardiovascular Surgery and Transplantations Brno (CKTCH) and find out the possibility of influencing the incidence of this complication by augmentation of primary sternal closure. Methods: Based on data from the National Cardiac Surgery Registry of Czech Republic, we retrospectively analyzed the risk factors of deep sternal wound defects in patients operated at the CKTCH between 2006 and 2011. Based on this results we designed a prospective study protocol, that was used for high risk patients in 2012-2013. In this group the primary sternal closure was performed by augmented technique. These patients were then statistically compared with patients with classic wire cerclage. Results: Retrospective analysis - we analyzed 6728 patients with 154 deep sternal wound defects (2.29%). We recorded higher mortality rate in a group of patients with DSWI (13% vs. 4%) and length of hospital stay (41.2 days vs. 14.5 days). Preoperative risk factors were as follows: BMI above 29.2 kg / m2, preoperative hemodynamic instability and a history of pulmonary disease. Perioperative risk factors: age over 65 years, LIMA harvesting (such as bypass graft for surgical myocardial revascularization), acute type of surgery and extracorporeal circulation time over 100 minutes. Statistically significant postoperative risk factors were: paraparesis, sepsis and infective complications, length of stay in the ICU over 72 hours, and postoperative psychosyndrome. Prospective analysis - in the group of 105 high-risk patients (based on the results of the scoring system and randomization) with augmentation by ZipFix sternal closure system, there was a deep poststernotomic defect in 1 case (0.95%). In the control group of 486 high-risk patients with sternal closure by conventional wire cerclage we found 20 patients with this serious complication (4.12%). Conclusion: In the first phase of our study we analyzed the risk factors of deep sternal wound defects at CKTCH Brno. Using augmentation techniques of sternal closure in a second phase we reduced incidence of DSWI, but our results are considered to be not statistically significant.
