Use of the McKenzie concept in the treatment of the vertebrogenic disorders of the lumbar spine

Abstract:

Cílem práce je přispět k řešení problematiky léčby vertebrogenních poruch v oblasti bederní páteře s využitím konceptu McKenzie. Pro svou bakalářskou práci jsem zvolila metodu kvalitativního výzkumného charakteru, kterou jsem zpracovala formou kazuistiky. Jako metodologické nástroje jsem zvolila rozhovor, pozorování a testování. Provedla jsem vstupní kineziologický rozbor pacientky aspekcí, dále jsem pacientku vyšetřila dle vstupního vyšetřovacího spisu pro bederní páteř konceptem McKenzie. Vedle anamnézy, funkčních aktivit a určení VAS škály bolesti, jsem také vyšetřila reakci symptomů na zátěž. Na základě vyšetření jsem stanovila předběžnou diagnózu asymetrický unilaterální derangement pod koleno, kterou jsem během další terapie potvrdila. K terapii jsem nejprve využila flekční princip s laterální komponentou a později princip extenční. Autoterapii vykonávala pacientka pravidelně každé 2-3 hodiny po 10 opakováních. Důležitou součástí léčby byla edukace pacientky, zaměřená na korekci držení těla a vyloučení agravačních faktorů. Terapie byla realizována v rozsahu 7 návštěv s frekvencí 2-3x týdně.

Abstract:

The aim of the paper is to help handle the problematic of lumbar spine vertebrogenic disorders therapy using the McKenzie concept. For my Bachelor´s Thesis I chose qualitative research work in aform of acase study.The utilized metodological tools are an interview, observation and testing. Duering the inicial examination, I performed an initial kinesiological examination by sight on the patient. Also I examined the patient using the McKenzie concept based on the initial examination file on lumbar spin. Apart from the anamnesis, functional activites and determioning the VAS pain lumbar spine scale score, I also examined the symptom response to stress. Furthermore I detected the limitation to the range of motion and response to repeated movements . Based on the examination I determined a preliminary diagnosis of asymmetric unilateral derangement below the knee, which I confirmed during the further therapy. In the terapy,I first employed the flection rotation principle with lateral component and later also extension principle. The patient practiced autotherapy regularly every 2-3 hours, doing 10 repetitions. An important part of the therapy was education of the patient, focused on the correct posture and elimination of aggravating factors. Therapy was carried out in the range of 7 visits with frequency of 2-3 times a week. Results: The Mechanical baseline was improved during therapy from significantly limited extension and moderately limited flexion together with the right-side lateral shift to no limitation in all direction. Thomayer distance shifted from 46cm to 6cm.The Functional baseline also showed signs of improvement after therapy. A prolonged walking is no longer causing any issues to the patient, she can now manage an hour long walk in contrast to the patient´s initial issuews with a 5 minute WALK. The VAS pain scale score changed from 5 to 0. The nerve root iritation radiating to the left leg was elilinated. Conclusions: Despite the initial issues, the patient is now able to perform standard daily tasks and engage in sports without being limited by pain in any way.
