Nursing process in care of patient after cardiac surgery

Abstract:

Bakalářská práce je zaměřena na ošetřovatelský proces v péči o pacienta, který podstoupil kardiochirurgickou operaci. Cílem bakalářské práce je sestavení názorného plánu péče, který se vztahuje ke konkrétním potřebám jedince a vychází z jednotlivých ošetřovatelských diagnóz NANDA-I taxonomie II. Bakalářská práce je rozdělena na dvě části - na část teoretickou a část praktickou. Teoretická část práce se člení na dvě hlavní kapitoly. První kapitola je věnována historii kardiochirurgie, operačním přístupům k srdci, mimotělnímu oběhu, indikací a operačnímu postupu při aortokoronárním bypassu. Druhá kapitola se věnuje předoperační přípravě pacienta, intraoperační péči, pooperační péči a také pooperačním komplikacím. Praktická část je soustředěna na plán ošetřovatelské péče o pacienta, který podstoupil aortokoronární bypass. Plán ošetřovatelské péče byl sestaven na základě ošetřovatelských diagnóz vycházejících z NANDA-I taxonomie II. Na základě získaných informací a dle NANDA-I taxonomie II bylo u pacienta stanoveno 12 ošetřovatelských diagnóz. K jednotlivým diagnózám byly vytvořeny cíle, ošetřovatelské intervence, byla popsána jejich realizace a také hodnocení ošetřovatelské péče, která byla poskytována.

Abstract:

The bachelor thesis is focused on the nursing process when taking care of a patient who underwent a cardiac surgery. The aim of this thesis is to create an illustrative care plan that relates to specific needs of an individual and that is based on individual nursing diagnoses of NANDA-I taxonomy II. The bachelor thesis is divided in two parts - i.e. the theoretical and the practical part. The theoretical part consists of two main chapters. The first chapter is devoted to the history of cardiac surgery, surgical approaches to the heart, extracorporeal circulation, indication and surgical procedure for aortocoronary bypass. The second chapter deals with the preoperative patient preparation, intraoperative care, postoperative care and postoperative complications. The practical part focuses on the care plan of a patient who underwent aortocoronary bypass. The care plan was based on nursing diagnoses NANDA-I taxonomy II. Based on the information obtained and NANDA-I taxonomy II, 12 nursing diagnoses were determined for the patient. For each of the diagnoses, individual aims and nursing interventions were set, as well as their realizations and evaluations of the nursing care were described.
