Quality of life in children with congenital heart defects

Abstract:

Bakalářská práce se zabývá problematikou kvality života dětí s vrozenými srdečními vadami. Hlavním cílem bylo vyhledat, analyzovat a sumarizovat aktuální vědecké poznatky o vnímání kvality života těchto pacientů, přičemž důraz je kladen na komparaci konkrétních diagnóz, srovnání se zdravými vrstevníky a zhodnocení vlivu operačních intervencí. Studie byly vyhledány systematickou rešeršní strategií za užití elektronických databází PubMed, CINAHL, EBSCO, ScienceDirect a Medvik pomocí předdefinovaných zahrnovacích a vylučovacích kritérií. Do práce byly zařazeny studie v českém a anglickém jazyce, publikované v letech 2016-2025. Extrakce, analýza a syntéza dat proběhly na základě stanovených 3 dílčích cílů. Po extrakci dat bylo zahrnuto 15 studií. Teoretická část definuje vrozené srdeční vady, jejich klasifikaci na kritické a nekritické a popisuje metody měření kvality života pomocí standardizovaných dotazníků (např. PedsQL, KINDL, DISABKIDS). Empirická část popisuje a porovnává výsledky vybraných studií. Výsledky analýzy ukazují, že kvalita života dětí s VSV je ovlivněna závažností vady, přítomností komorbidit a věkem pacienta. Významné deficity ve fyzickém ipsychosociálním fungování jsou patrné zejména u syndromu hypoplastického levého srdce. U ostatních typů VSV jsou přitom výsledky výrazně příznivější, kdy studie popisují kvalitu života srovnatelnou, nebo dokonce vyšší než u zdravé populace. Po kardiochirurgických operacích vykazují pacienti s paliativní korekcí nižší skóre kvality života ve srovnání s dětmi po kompletní biventrikulární korekci.

Abstract:

This bachelor thesis addresses the issue of quality of life in children with congenital heart defects (CHDs). The primary objective was to identify, analyze, and summarize current scientific knowledge regarding the perceived quality of life in these patients, with anemphasis on comparing specific diagnoses, contrasting them with healthy peers, and evaluating the impact of surgical interventions. Studies were identified through a systematic search strategy using electronic databases including PubMed, CINAHL, EBSCO, ScienceDirect and Medvik, based on predefined inclusion and exclusion criteria. The thesis included studies published in Czech and English between 2016 and 2025. Data extraction, analysis, and synthesis were conducted according to three established sub-objectives. Following the data extraction process, 15 studies were included in the final analysis. The theoretical part defines congenital heart defects, their classification into critical and non- critical categories, and describes methods for measuring quality of life using standardized questionnaires (e.g., PedsQL, KINDL, DISABKIDS). The empirical part describes and compares the findings of the selected studies. The results of the analysis indicate that the quality of life in children with CHDs is influenced by the severity of the defect, the presence of comorbidities, and the patient's age. Significant deficits in both physical and psychosocial functioning are particularly evident in patients with hypoplastic left heart syndrome. Conversely, results for other types of CHDs are considerably more favorable, with studies describing a quality of life comparable to, or even higher than, that of the healthy population. Following cardiothoracic surgery, patients with palliative correction exhibit lower quality of life scores compared to children who have undergone complete biventricular repair.
