The skull base pathology - the skull base border quality

Abstract:

Patologie spodiny lebeční - kvalita hranice baze lební Cílem práce je stanovení kvality tvrdé pleny mozkové v oblasti spodiny lebeční dle lokalizace a v oblasti přední jámy lební, která nejvíce souvisí s dutinami lebky, studium nejcitlivejší oblasti – lamina cribrosa. Ke studiu je využita metoda histologického vyšetření tvrdé plena ve 3 navzájem kolmých osách, zjištění jejích biomechanických vlastností a srovnání s jedinou studií Melvina a kolektivu z roku 1975 [131]. Makroskopicky a mikroskopicky byla provedena studie i oblasti lamina cribrosa ve speciálním barvení. Studie prokázala, že vlastní lamina cribrosa je tedy kostí labyrintového typu a způsob průniku struktur (cév a nervů) intrakranio-extrakraniálně. („lamina cribrosa labyrinthiformis“ či „lamina cribrosa labyrinthi simile“). Na čichovém nervu byla prokázana existence Fondánovi undulace. Byl popsán vznik plexus olfactorius již při průchodu kostní strukturou - lamina cribrosa v jejím labyrintovém systému kanálků a vytvoření tzv. plexus venosus olfactorius. Oblast lamina cribrosa je skutečně nejcitlivějším místem v oblasti spodiny lebeční, jedná se o locus minoris resistentiae. Tvrdá plena na konvexitě je nejpevnější a nejtužší, pevný obal mozku, směrem ke spodině lebeční dochází k jejímu ztenčení, je jemnější, méně tuhá a pevná. Na konvexitě je dura 2x tužší, 2x méně elastická než na spodině lebeční. V oblasti konvexity jsou rozdílné vlastnosti v rovině koronární a sagitální (jedná se o statistický významný rozdíl), a rovněž v oblasti přední jámy lební, tedy na bazi. Sagitálně je na konvexitě i na bazi přední jámy lební dura tužší, méně tažná, méně elastická. Srovnání histologického nálezu 3D a biomechanických vlastností tvrdé pleny spolu velmi úzké souvisí a koreluje. Biomechanické studie při tahové zkoušce poukazují i na vnitřní složení materiálu. Tvrdá plena na bazi vytváží bariéru intrakrania a extrakraniální oblasti, která je limitně pevná a vytváří ochranu pro prostup struktur (cév a nervů) skrze bazi speciálním uspořádáním kolem nich, aby bylo zachováno oddělení intrakrania od okolí. Tvrdá plena má i funkční význam (hemodynamický, termodynamický, liquorodynamický, imunologický, elektromagnetického kontinua), ale i mechanický význam ochrany mozku.

The skull base pathology - the quality of skull base border The work goals are determination of the dural quality of the skull base area under localisation and into anterior skull base fossa, which the most connected with paranasal spaces, the study of the most sensitivity area - lamina cribrosa. The method of histology of dural sheats into 3 mutually perpendicular coordinate axis, finding out their biomechanic characteristics and correlation with Melvin and cooworkers study from 1975y. [131]. The study was performed in area lamina cribrosa macroscopically and microscopically with special colour technics. The study proves, that the lamina cribrosa is oss of labytinth type and way of structures (vessels and nervs) intracranial-extracranially („lamina cribrosa labyrinthiformis“ or „lamina cribrosa labyrinthi simile“). The exit of Fondan´s undulations was proved on olfactory nerves. The form of plexus olfactorius was described during way through osseous structures – lamina cribrosa – into osseous labyrinth channel system and so-call plexus venosus olfactorius creates. The area of lamina cribrosa is exactly the most sensitivity part of skull base, it is “locus minoris resistentiae”. The dura on convexity is the strongest and the most stiff, firm cover of brain, to the skull base way comes to this thin, is finner-grained, less strong and less stiff. On convextiy there is dura 2time strong, less elastic then on skull base. The different quality are into coronary nad sagitally plane in convexity (it is statistically significant diference) and to in anterior skull base fossa. Sagitally is dury stronger, less elastic, less drawable on convextiy and anterior skull base fossa. The comparison of 3D histology findings and biomechanic characteristics of dura are very narrow related and correlated. The biomechanical study duriny tensile test indicate to inner structure of material. The dura mater on skull base form the barrier intracranial and extracranial parts, which is thresholdly strong and form protection for crossway structures (vessels and nerves) through skull base with special arrangement surround their, to preserve separation intracranial space from surround. The dura has functional points (hemodynamic, thermodynamic, liquorodynamic, imnunological, electromagnetic continnuum), but mechanical point of brain protection too.
