Psychotherapy in the complexity paradigm: uncovering the power of network analysis in clinical samples

Abstract:

Tato disertační práce se zaměřuje na využití matematických nástrojů jako je teorie grafů nebo síťová analýza dat v rámci psychoterapeutického výzkumu. Práce si klade za cíl poukázat na užitečnost těchto metod pro sérii různých výzkumných otázek a představit tak smysluplnou paletu nástrojů pro kvantitativně orientované výzkumníky v oboru psychoterapie. V rámci teoretického úvodu se práce věnuje komplexním systémům a důležitosti propojenosti dílčím prvků daného systému - změny v chování dílčích částí mohou ovlivnit celý systém. Komplexní systémy jsou často nelineární, kdy i velmi malé rozdíly v počátečních podmínkách mohou vést k zásadním změnám v celkové trajektorii vývoje celého systému. Síťový přístup (network) je podmnožinou komplexního přístupu a reprezentuje vztahy mezi jednotlivými prvky v rámci systému jako síť propojených proměnných. Tato disertační práce zahrnuje pět studií. První studie ověřuje validitu české verze Group Cohesiveness Scale (GCS) a přichází s rozdělením faktorové struktury na afektivní a behaviorální složku skupinové koheze. Disertační práce má navíc oproti publikované studii přidanou kapitolu ohledně porovnání přístupu latentních faktorů se síťovým přístupem. Druhá studie zkoumá psychometrické vlastnosti českého překladu Emotion Regulation Skills Questionnaire (ERSQ). Faktorové skóry jednotlivých dimenzí schopností emoční regulace jsou následně vloženy do longitudinálního síťového modelu pro panelová data k zhodnocení prediktivní validity nástroje ve vztahu k wellbeingu. Pomocí síťového modelu lze usuzovat na existenci dvou hlavních klastrů emoční regulace: percepční a modulační. Centrální roli v této síti hraje modifikace negativních emocí. Třetí studie sleduje změny ve wellbeingu, depresivitě a úzkostnosti v rámci baseline měření v porovnání s měřením na konci intervence u pacientů léčby založené na větším množství komponentů včetně terapie. Pro zhodnocení efektu léčby na změnu v průměrech sledovaných proměnných je použit standardní způsob analýzy dat: lineární model se smíšenými efekty. K tomu je použit také položkový síťový model ke zjištěný změn na úrovni vazeb mezi jednotlivými položkami všech tří nástrojů (změny v síťové struktuře). Studie přináší „extrémní evidenci“ ve prospěch stanovaných hypotéz (interpretace Bayes faktorů). Co se týče sítí, struktura sítí se mírně proměňuje. Na konci léčby získávají položky úzkostnosti mírně centrálnější postavení na úkor položek depresivity. Nicméně, celková hustota vztahů mezi položkami se neproměňuje. Čtvrtá studie zjišťuje mechanismy změny vedoucí k vyššímu wellbeingu na úrovni celého vzorku pacientů léčby založené na větším množství komponentů včetně terapie. Studie se zaměřuje na jednotlivé domény pocházející z více obecných teoreticky podložených mechanismů změny: emoční regulace, interoceptivní uvědomění a přijetí symptomu. K tomu je explorativně použit longitudinální síťový model pro panelová data. Model je následně očištěn o nesignifikantní vztahy mezi faktory (uzly v modelu) a vykazuje dobrý fit. Tento typ modelu generuje tři typy sítí (temporální, kontemporální a mezi-subjektovou). Temporální síť je založená na prediktorech vždy z předchozího bodu měření (lag-1). Modifikace negativních emocí má zásadní postavení v síti mechanismů změny a je přímo temporálně vztažená k wellbeingu. Další mechanismy, které wellbeing ovlivňují přímo jsou připravenost na konfrontaci s negativními emocemi, přijetí ve smyslu užívání si aktivit nehledě na symptom a důvěra ve vlastní tělesné signály. ace a opak obávání se, když si člověk uvědomí své tělesné signály. Přijetí negativních emocí vykazuje negativní vztah k wellbeingu. Využití síťové analýzy bylo užitečné k vytvoření nových hypotéz pro budoucí výzkum a praxi. Výsledky studie dále mírně narušují tradiční kategorizaci proměnných do závisle proměnných a mechanismů změny v psychoterapii a navrhuje více dynamický pohled na terapeutický proces. Pátá studie provádí rešerši šíře metod z oblasti komplexní vědy nebo teorie dynamických systému využívané ve výzkumu psychoterapie a navrhuje, že psychoterapii lze chápat jako perturbovaný dynamický systém. Vyhledávání primárních studií je provedeno v databázích EBSCO a Web of Knowledge. Extrahované informace obsahují použité matematické metody, popis specifických dynamických modelů, psychoterapeutických jevů, etc. Po sledování naplnění vylučujících a zahrnujících podmínek u velkého množství abstraktů a 4194 plných textů je identifikováno 41 studií publikovaných mezi lety 1990 a 2021. Použité metody často zahrnují měření dynamické komplexity nebo chaoticity. Komputační a simulační studie často zahrnují ordinálně diferenciální rovnice prvního řádu a typicky se zaměřují na popis časového vývoje vzájemných vlivů mezi terapeutem a pacientem v terapii. Pokud studie zahrnuje také empirická data, obvykle se jedná o případové studie s vysoce časově intenzivními daty (dynamika v rámci sezení). Rešerše poskytuje spíše deskriptivní syntézu současného stavu aplikace metod z komplexní vědy do výzkumu psychoterapie. Shrnutí: těchto pět studií zdůrazňuje důležitost zapojení větší palety různých metod (s důrazem na zapojení síťové analýzy) k posuzování různých otázek typických pro psychoterapeutický výzkum – např. validace nástrojů měření, efektivita léčby, mechanismy změny a modelování psychoterapie jakožto dynamického systému. Využití těchto metod může navýšit porozumění propojení mezi jednotlivými prvky psychoterapeutických systémů a potenciálně zvýšit efektivitu psychoterapeutické léčby. Výzkumníci a terapeuti jsou v závěru povzbuzeni ke spolupráci pro zapojení zpětné vazby (zjištěné díky těmto metodám) do léčby.

Abstract:

This dissertation focuses on the application of mathematical tools such as network theory or network analysis in psychotherapy research. The aim is to demonstrate the usefulness of these methods in various research questions and to draw conclusions about their meaningfulness in the potential daily toolkit of quantitative psychotherapy researchers. The theoretical introduction of this dissertation highlights the inherent interconnectedness of complex systems, where changes in a part of the system can affect the entire system. Complexity can be also recognized by nonlinearity when even very small differences in initial conditions can lead to major changes in the overall trajectory of the dynamic development of the whole system. The usefulness of network theory, an approach driven by complexity science that represents the relationships between individual elements within a system as a network of interconnected variables, is emphasized. The dissertation comprises five studies. The first study validates the Group Cohesiveness Scale (GCS) in the Czech language using factor and network models, revealing two domains of group cohesion: affective and behavioral. The first study also demonstrates that the Czech version of the GCS is a valid and reliable tool for measuring both aspects of group cohesiveness. A cross-sectional network model on the item level has been estimated to compare latent variable and network approaches in gaining psychometric insights. The second study examines the psychometric properties and longitudinal network structure of the Czech translation of the Emotion Regulation Skills Questionnaire (ERSQ). The study finds that the Czech translation of the ERSQ is a psychometrically sound instrument comparable to the original version, even though the instrument is multi-dimensional. Individual factor scores of ERSQ domains were modeled as a longitudinal network indicating the existence of perceptive and modulatory clusters of emotional regulation skills and revealing the central role of modification of negative emotions in emotional regulation. The network model was primarily applied to investigate the predictive validity of the scale toward wellbeing. The third study assesses pre-post changes in wellbeing, depression, and anxiety outcomes in patients undergoing a multicomponent treatment using a combination of traditional mixed models by investigating changes in mean severity of symptoms (together with increases in wellbeing), and network methodology by investigating mainly changes in a network structure. The study finds extreme evidence in favor of hypotheses suggesting pre-post psycho-therapy effectiveness. Network structure showed a mild reduction of depression importance while an increase in anxiety importance. However, the overall density of associations did not change after treatment. The fourth study examines the mechanisms of change of wellbeing at the whole adult patient sample level in group-based psychodynamic multicomponent treatment. The study focused on domains from more general hypothesized mechanisms of emotion regulation, interoceptive awareness, and acceptance. A lag-1 longitudinal network model was employed for exploratory analysis of the panel data. The pruned final model demonstrated a satisfactory fit and included temporal, contemporaneous, and between-person networks. The most central node was again the modification of negative emotions. Mechanisms that were temporally positively associated with wellbeing included modification, readiness to confront negative emotions, activity engagement, and trust in the own bodily signals. Acceptance of negative emotions showed a negative association with wellbeing. Moreover, noticing, bodily sensations, not worrying, and self-regulation contributed indirectly to changes in wellbeing. variables into either outcomes or mechanisms in psychotherapy and suggested a more dynamic view of the therapeutic process. The fifth study reviews the variability of dynamic systems theory and complexity science methods applied in psychotherapy research, suggesting that psychotherapy can be modeled as a perturbed dynamic system. A primary studies search was conducted in the EBSCO and Web of Knowledge databases, extracting information about the employed mathematical methods, descriptions of specified dynamic models, temporal dynamics, psychotherapy phenomena investigations, and other types of empirical data. After screening 38,216 abstracts and 4,194 full texts, 41 studies published from 1990 to 2021 were identified. The employed methods typically included measures of dynamic complexity or chaoticity. Computational and simulation studies most often employed first-order ordinary differential equations and typically focused on describing the time evolution of client-therapist dyadic influences. Eligible studies with empirical data were usually based on single- or multiple-case studies and focused on data with high time intensity of within-session dynamics. The review provides a descriptive synthesis of the current state of the proliferation of complexity science methods in the psychotherapy research field. In sum, these studies highlight the importance of employing diverse methodologies (mainly network analysis) to investigate various facets of psychotherapy research, such as psychometric validation, treatment effectiveness, mechanisms of change, and modeling psychotherapy as a dynamic system. The use of these methods can enhance our understanding of the interconnectivity between individual elements within a system and potentially increase the effectiveness of psychotherapy treatment. The review of dynamic systems theory and complexity science methods used in psychotherapy literature further emphasizes the potential for these methods to inform our understanding of psychotherapy as a dynamic system. Researchers and therapists are encouraged to work together to incorporate network feedback into treatment.
