Educational standard of nursing care - Preparation of patient before carotid surgery in cervical block

Abstract:

Karotická endarterectomie je standardní léčbou závažných symptomatických a asymptomatických stenóz karotických tepen. Je to preventivní operace s cílem snížit riziko cévní mozkové příhody v důsledků embolie z ateromového plátu. Z poznatků dle Evidence-Based Practice provádíme operaci v regionální anestezii, která umožňuje technicky nenáročné monitorování neurologického stavu pacienta během výkonu s možností časného odhalení mozkové ischémie. Výhodou je i lepší hemodynamická stabilita v perioperačním a pooperačním období. Teoretická část bakalářské práce je zaměřena na problematiku operačních výkonů na krčních tepnách. Cílem bakalářské práce je vypracování edukačního standardu ošetřovatelské péče pro potřeby všeobecných sester anesteziologické ambulance, ARO a chirurgického oddělení Nemocnice Podlesí a.s. Třinec. Správnou edukací chceme pacienta informovat o předoperační přípravě, volbě anestezie, o průběhu operace a pooperační péči. Pro pacienty, kteří mají podstoupit operaci na krčních tepnách, byla vypracovaná brožura, kde najdou všechny potřebné informace a pokyny spojené s operací.

Abstract:

Carotid endarterectomy is the standard treatment for severe symptomatic and asymptomatic carotid artery stenosis. It is a preventive operation in order to reduce the risk of stroke due to embolism from atheromatous plaque.According to Evidence Based Practice, the surgery is performed under regional anesthesia that allows simple monitoring of neurological status of the patient during the procedure with the possibility of early detection of cerebral ischemia. The advantage is also better haemodynamic stability in the perioperative and postoperative period. The theoretical part of the thesis is focused on surgical procedures in carotid arteries.The objective of the bachelor thesis is to develop educational standard of nursing care for the requirements of nurses of preanesthesia evaluation service, ICU and the Department of Surgery of Hospital Podlesí a.s. Třinec. With appropriate education, we want to inform the patients about the preoperative preparation, the choice of anesthesia, about the surgical procedure and the postoperative care. For the patients who undergo carotid surgery, brochure has been prepared in which they will find all the necessary information and instructions for the operation.
