Abstract:

Anotace Diplomová práce popisuje pokus o změnu systému zdravotní péče ve Spojených státech amerických. Mnoho politiků a veřejných činitelů se v minulosti snažilo s menším či větším úsilím standardizovat péči a poplatky za lékařské služby, ovšem s malým úspěchem. Za částečný úspěch lze považovat programy Medicare, Medicaid a další, které ovšem měly a mají za následek plýtvání prostředky a narůstání nákladů pro federální a státní rozpočty. Zatím poslední pokus o změnu v podání prezidenta Baracka Obamy došel nejdále ze všech: nový systém byl protlačen americkým zákonodárným procesem a ustanoven zákonem. Práce je rozdělena do šesti základních kapitol. Úvod nastiňuje základní otázky problematiky a obsah kapitol. První kapitola se zaobírá jedinečností americké společnosti a jejím rozdílným chápáním individuálního versus kolektivního vidění světa v porovnání s ostatními rozvinutými státy dnešního světa, ve kterých systém veřejného zdravotního pojištění funguje. Ve druhé kapitole je shrnuta historie zdravotní péče v USA do poloviny dvacátého století s důrazem na její legislativní a ekonomicko-sociální stránku. Kapitola třetí popisuje novodobou historii pokusů o změnu a nápravu systému v druhé polovině dvacátého století, jejich vliv na myšlení občanů a následnou větší podporu zmiňované reformy. Další dvě kapitoly popisují snahu mladého a ambiciózního prezidenta Baracka Obamy, který za pomoci demokratické většiny v Kongresu včlenil novou reformu do života Američanů. Zároveň popisují implementaci nového zákona a s tím spojené počáteční problémy a zmatky. Poslední kapitola mluví o překážkách kladeným různými zájmovými a byznys skupinami, jejichž snahou je zvrácení procesu nové reformy. V neposlední řadě také pojednávají o tlaku nové administrativy presidenta Donalda Trumpa ve spojení s republikánskou většinou v kongresu na zrušení tohoto systému.

Abstract:

Annotation The presented Master’s thesis describes an attempt to change the U.S. health care system. Many politicians and public figures had been trying to break a deal among Americans in order to standardize their fractured insurance policies and health care benefits; however, there was not much success. As a partial improvement has been the establishment of federal government programs such as Medicare, Medicaid and others, nevertheless, those have put an enormous pressure on federal and state budgets while also increased the ever-present wasteful spending. So far, the last reform ACA (Affordable Care Act), nicknamed “ObamaCare”, has been already driven through the legislative process and made a law. The work is divided into six main chapters. The introduction presents basic notions about the topic and lays out the content of the following chapters. The first chapter describes the uniqueness of American society and its different understanding of individual versus collective view of the world in contrast with other developed countries where the public health insurance system works. The second chapter reveals the history of health care in the U.S. until the 1960s with emphasis on its legislative and social-economic aspects. The next chapter describes attempts at a change in the second half of the twentieth century, their influence on citizens and the subsequent wider support of the reform. The next two chapters present an overview of the successful pursuit of a young and ambitious president Barack Obama who has, with the help of the majority of his fellow Democrats in the U.S. Congress, pushed the new health care reform through the legislative process, and incorporated it into the lives of many Americans. They also describe the new law’s implementation and the initial problems and confusion. At last, the final chapter includes several obstacles being placed against this reform by various business and interest groups in the sake of slowing down or even stopping it. In addition it describes the first steps of the new President Donald Trump’s administration backed by both parts of Republican majority Congress who are eager to repeal the law.
