Some etiopathogenic relationships of atopic dermatitis

Abstract:

Atopická dermatitída (AD) je komplexné ochorenie, ktoré je výsledkom vzájomnej interakcie genetickej predispozície, vplyvov vonkajšieho prostredia, neuro-imunologickej dysbalancie a porušenej epidermálnej bariéry. Genetické štúdie identifikovali niekoľko génov a génových lokusov asociovaných s AD. Jedným z perspektívnych kandidátsky génov je i gén pre eotaxín, lokalizovaný na chromozóme 17q.21. Eotaxin je chemokín, ktorý hrá dôleţitú úlohu u AD ako chemoatraktant eozinofilov. Ďalším dôleţitým etiologickým faktorom AD je porucha epidermálnej bariéry, ktorá spôsobuje pokles hydratácie koţe zvýšenými transepidermálnymi stratami vody (TEWL), tieto straty je moţné objektívne zmerať pomocou TEWL metra.Naše výsledky poukazujú na to, ţe eotaxín a lokus 11q13 hrajú dôleţitú úlohu v patogenéze AD a iných komplexných ochorení. Detekcia polymorfizmov v týchto genomických oblastiach sa zdá byť sľubnou cestou pre nájdenie klinicky validných biomarkerov pre AD. Meranie TEWL je veľmi dôleţitý parameter hodnotiaci stav koţnej bariéry objetívnou a noninvazívnou cestou. Výsledky našich meraní poukazujú na primárny defekt epidermálnej bariéry u AD.

Abstract:

Atopic dermatitis (AD) is a multifactorial disease. There is an interplay between strong genetic and environmental factors, metabolic and immunologic mechanisms, and a defect of the epidermal barier in the pathogenesis of AD. Genetic studies identifed several genes and genic loci associated with AD, and novel insights into its pathogenesis. One of the eotaxin genes is localised in chromosome 17q21. Eotaxin may play an important role in atopic dermatitis (AD) as a potent immunomodulatory agent. Another important factor in the pathogenesis of AD is disruption of the epidermal barrier, as indicated by a reduction in skin hydration, and an increase in transepidermal water loss (TEWL). It is possibble to measure this objectively using a TEWL meter. Our results suggest that the eotaxin gene and locus 11q13 play an important role in the pathogenesis of AD, and other complex diseases. The detection of the polymorphisms in these genomic localities in patients with AD seems to be a promising way of finding clinically valid gene biomarkers for this disease The TEWL measurement is the most important parameter for evaluating the efficiency of the skin water barrier in an objective and non-invasive way. The result of the study may indicate a primary defect in the epidermal barrier in AD.
