Endoluminal radiofrequency ablation of bile ducts

Abstract:

Paliace tumorů postihujících žlučové cesty patří dlouhodobě k problematice, která nedosahuje uspokojivých výsledků v celosvětovém měřítku. Obstrukční ikterus komplikuje jak primární terapii nádorového onemocnění, tak další léčbu při progresi nebo rekurenci onemocnění. I přes možnosti léčby obstrukce žlučových cest chirurgickou či endoskopickou cestou je stále velké množství pacientů, u kterých je perkutánní drenáž jedinou možností terapie cholestázy. Optimální paliativní metodou pro tyto pacienty je zavedení metalického kovového stentu. Problémem je jeho časná okluze a rovněž řešení okluze metalického stentu. V práci Endoluminální radiofrekvenční ablace žlučových cest jsme se zaměřili na výzkum techniky ablace pomocí katetru endoHPB jednak v indikaci ablace žlučových cest před zavedením kovového samoexpandibilního stentu a jednak při řešení okluze stentu. V ex vivo studii jsme prokázali lineární závislost makroskopicky hodnoceného efektu a hloubky ablace od výkonu generátoru. Hodnotili jsme vliv pomalého proplachu žlučových cest na průběh ablace, kde nejvýraznější i když ne signifikantní rozdíl nacházíme u ablace při 5 wattech. Optimální výkon generátoru dle ex vivo testování stanovujeme na 10 W. V klinické prospektivní randomizované studii jsme neprokázali vliv výkonu endoluminální ablace na délku přežití pacientů, nicméně ve skupině pacientů podstupujících ablaci jsme našli nižší míru okluze stentu, časné okluze do 2 měsíců od zavedení stentu jsme nezaznamenali. Výkon navíc nebyl spojen s vyšší mírou komplikací. Při řešení obstrukce stentů endoluminálním katetrem v bipolárním módu jsme prokázali účinnost metody, sekundární průchodnost stentu byla v průměru 4,3 měsíce. Vzhledem k ne zcela uspokojivým výsledkům vidíme potenciál metody v monopolárním způsobu ablace obstrukce stentu. V ex vivo testování pomocí termometrických a termografických studií jsme navrhli optimální protokol takto provedené ablace se zřetelem na bezpečí pacienta.

Abstract:

Tumors of the pancreas and biliary tract causing biliary obstruction present a major medical problem. Obstructive jaundice complicates both the primary cancer therapy and further treatment when tumor progression or disease recurrence occurs. Despite surgical or endoscopical treatment options for biliary decompression, there is still a large number of patients in whom percutaneous drainage is the only therapeutic method for cholestasis. Optimal palliative method for these patients is the insertion of metal stent. The problem is the early occlusion and also treatment of occluded metal stent. The study Endoluminal radiofrequency ablation of bile ducts is focused on research of ablation techniques with catheter endoHPB - both in indication of ablation before the self-expandible metal stent placement and in dealing with stent occlusion. In the ex vivo study, we have demonstrated a linear dependence of macroscopically evaluated effect and depth of ablation from the output power of generator. We evaluated the effect of slow flushing of the bile ducts on the course of ablation, where the most difference is in ablation at output of 5 watts (not statistically significant). The optimal power output of generator according to ex vivo testing we indentify as 10 W. In the prospective randomized clinical study we did not prove the effect of endoluminal ablation on patients survival, however, in the group of patients undergoing ablation, we found a lower rate of stent occlusion and early stent occlusion (up to two months of stent placement). In addition, the intervention was not associated with higher rates of complications. In the treatment of stent obstruction by endoluminal bipolar catheter ablation we have demonstrated the effectiveness of the method, the secondary stent patency was on average 4.3 months. Considering the unsatisfactory results, we see the potential of the method in the use of monopolar mode of ablation. In ex vivo testing using thermometric and thermographic studies we have proposed an optimal protocol of ablation with regards to patient's safety.
