Physiotherapy with focus on stereotypes of walking among paretic patients suffering from multiple sclerosis

Abstract:

Bakalářská práce se zabývá poruchami chůze u paretických pacientů s roztroušenou sklerózou (RS). Cílem práce bylo zjistit a použít dostupné možnosti fyzioterapie, které mají možný vliv na patologický stereotyp chůze právě u paretických pacientů s RS. V první části práce je popsaná charakteristika onemocnění. Zahrnuje základní informace o etiopatogenezi, klinickém obrazu, diagnostice a průběhu nemoci, dále popisuje fyziologii chůze, včetně kinematiky dolních končetin, problematiku a mechanismy ovlivňující chůzi u paretických pacientů s RS a vybrané metody pro její ovlivnění. V druhé části práce jsou popsány dvě kazuistiky pacientů s RS, u kterých jsem se snažil pomocí vybraných rehabilitačních metod ovlivnit patologické stereotypy chůze. Jednou z hlavních fyzioterapeutických intervencí byl senzomotorický trénink za využití moderního diagnosticko-terapeutického přístroje Zebris-Rehawalk? s cílem symetrizovat rotaci chodidel, délku a šířku kroku a přiblížit se symetrickému zatížení obou chodidel během stojné fáze chůzového cyklu. Individuální fyzioterapie byla uskutečňována 1x týdně přibližně 60 minut po dobu 3 měsíců u pacientů s RS v relaps-remitentní (EDSS 4,0) a sekundárně progresivní (EDSS 7,0) fázi. Výsledky práce ukazují, že kombinací senzomotorického tréninku a využití jednotlivých modulů, které nabízí přístroj Zebris-Rehawalk? lze u obou pacientů s RS zlepšit stabilitu ve stoji a při chůzi, dále se přiblížit symetrickému zatížení a rotaci chodidel ve stojné fázi chůzového cyklu. Co se týká využití přístroje u paretických pacientů s vyšším skóre dle Kurtzkého škály nebo jinou symptomatikou průběhu onemocnění, bylo by pro potvrzení jeho terapeutické účinnosti zapotřebí rozsáhlejší studie s větším počtem probandů.

Abstract:

The bachelor´s thesis deals with the topic of walking disturbance with MS (multiple sclerosis) patients with paresis. The goal of the thesis was to find out and use the physiotherapy possibilities which influence pathological walking stereotype with patients with MS. The first part describes disease characteristics. It includes the basic information on etiopathogenesis, clinical picture, diagnosing and disease course. It also describes walking physiology including lower limbs kinematics, the problems and mechanisms influencing MS patient with paresis walking and chosen methods how to influence them. The second part describes two patients with MS and their case history, where I tried (through chosen rehabilitation methods) to influence the pathological walking stereotype. One of the main physiotherapy interventions was the sensomotoric training using Zebris-Rehawalk device to symmetrize foot rotation, stride length and width and to approach both feet symmetric strain during the standing phase of a walk cycle. Individual physiotherapy was realized once a week 60 minutes during 3 months with patients with relapsing-remitting MS (EDSS 4.0) and secondary progressive MS (EDSS 7.0). The outcome shows that the combination of the sensomotrric training and using the individual modules through the Zebris-Rehawalk? device enables to improve the gait and walking stability and to approach both feet symmetric strain during the standing phase of a walk cycle. Using the device with the patients with the higher EDSS or with other symptoms in a more complex study could confirm its therapeutic possibilities.
