Mastectomy and reconstructive surgery in patients with BRCA mutation and breast cancer

Abstract:

Disertační práce podává přehled o epidemiologii karcinomu prsu ve světě a České republice. Jsou shrnuty historické a aktuální poznatky o operačních výkonech na prsech a adnexech u žen s BRCA 1,2 mutací a karcinomem prsu. Podrobněji jsou rozvedeny operační techniky mamární a onkoplastické chirurgie používané na našem pracovišti a doplněny vlastními schématy a obrázky autora. Dále jsou prezentovány výsledky zavedení programu okamžité rekonstrukce prsu u žen s BRCA 1,2 mutací a pacientek s karcinomem prsu do klinické praxe. V textu jsou uvedeny a zapracovány práce prezentující naše výsledky na světových kongresech (ESMO Berlín - Ann Oncol, ASRM – USA - Fertil Steril). Práce shrnuje kvantitativní výzkum informovanosti o okamžité rekonstrukci prsu u žen s prokázanou genetickou mutací BRCA 1,2 nebo s karcinomem prsu. Bylo prokázáno, že většina žen si přeje zachovat celistvost těla a v případě potřeby, je-li to možné, provedení okamžité rekonstrukce prsu.

Abstract:

This doctoral thesis dissertation provides an overview of the epidemiology of breast cancer in the world and the Czech Republic. Historical and current findings on breast and adnexal surgery in women with BRCA 1,2 mutations and breast cancer are summarized. The surgical techniques of mammary and oncoplastic surgery used at our workplace are explained in more detail. They are supplemented with the author's own diagrams and pictures. Furthermore, the results of the introduction of the program of immediate breast reconstruction in women with BRCA 1,2 mutations and patients with breast cancer into clinical practice are presented. Papers presenting our results at leading world congresses (ESMO Berlin – Ann Oncol, ASRM USA - Fertil Steril) are presented and incorporated in the text. The work summarizes quantitative research on awareness of immediate breast reconstruction in women with a proven BRCA 1,2 genetic mutation or with breast cancer. It has been shown that the majority of women wish to preserve the integrity of the body and, if necessary, to perform immediate breast reconstruction if possible.
