Abstract:

Předmětem předkládané disertační práce je spoluúčast pacientů v České republice. V roce 2008 významně vzrostla výše výdajů pacientů za zdravotní péči a je velmi pravděpodobné, že se celková spoluúčast bude zvyšovat i v budoucnu z důvodu posílení udržitelnosti financování zdravotní péče. Jednou z nejzávažnějších obav souvisejících se zvyšováním spoluúčasti je možné omezení přístupu ke zdravotní péči v důsledku neschopnosti za tuto zdravotní péči zaplatit. Jestliže tedy bude docházet k dalšímu zvyšování spoluúčasti pacientů, je třeba eliminovat negativní dopady právě na přístup ke zdravotní péči a chránit ohrožené skupiny pacientů. Prvním z cílů disertační práce je determinovat a zhodnotit dopady spoluúčasti pacientů a jejího zvýšení na zatížení rozpočtů domácností a finanční dostupnost zdravotní péče v České republice v letech 2007 – 2009. Za účelem dosažení definovaného cíle bylo stanoveno 5 pomocných výzkumných otázek. Druhým cílem je navrhnout souhrn doporučení týkajících se rozhodování o implementaci takových ochranných prvků do systému spoluúčasti, které minimalizují negativní dopady na přístup pacientů ke zdravotní péče. Výzkum vychází ze tří oficiálních průzkumů – Statistika rodinných účtů, šetření Životní podmínky (EU–SILC) a Evropské výběrové šetření o zdraví v České republice. Mezi použité výzkumné metody patří jak obecně vědní tak speciálně vědní metody. Disertační práce je tvořena 5 kapitolami. Výsledky výzkumu jsou prezentovány ve čtvrté kapitole, doporučení na základě zjištění jsou vyvozena v páté kapitole. Shrnutí všech výstupů jsou obsaženy v závěru práce. Výsledky prezentované v disertační práci syntetizují poznatky o dopadech spoluúčasti pacientů na finanční zatížení rozpočtů českých domácností a o omezení dostupnosti zdravotní péče. Velmi důležitým přínosem je definice potenciálně ohrožených skupin české populace, které by v případě dalšího zvyšování spoluúčasti byly zasaženy nejvíce. Disertační práce také přispívá k diskuzi o zavádění různých schémat spoluúčasti a poskytuje návrhy a doporučení pro tvůrce zdravotní politiky.

Abstract:

The subject of the present doctoral thesis is cost sharing of patients for health care with the focus on the Czech Republic. The level of out-of-pocket spending increased in 2008 and it is highly probable that cost sharing will further rise in the future due to the sustainability of health care financing. One of the most important concerns regarding increasing in cost sharing is the possible restriction of access to health care services caused by the inability to pay for health care. Therefore, if we agree on a further increase in cost sharing of patients, we have to eliminate the negative impact on access to health care and to protect the most vulnerable groups. The first objective of the doctoral thesis is to determine and estimate the impact of cost sharing and its increase in the health care system on the household’s burden and financial access to health care in the Czech Republic in years 2007 - 2009. There are five research questions to help us to achieve this objective. The second objective is to propose a set of recommendations related to the decision making about the implementation of protective features in the cost sharing system which minimise the negative effects on the financial access to health care. In order to achieve the defined objectives, the research is based on the data collected from three official surveys – The Household Budget Survey, The Survey on Income and Living Conditions and The European Health Interview Survey. The methods used in the research are both general and specialised scientific methods. The doctoral thesis consists of five chapters. Results of the research are presented in Chapter 4, applications of the findings and their transformation with regards to the recommendations for the Czech health care system are conducted in Chapter 5. Summary of all findings and recommendations is presented in the conclusion. The doctoral thesis and its results synthesise the knowledge about the impacts of cost sharing on the burden of Czech households and restriction of the access to health care. The most important contribution is the definition of particular groups of Czech population that could be affected the most in case of the further increase in out-of-pocket payments. The thesis also contributes to the discussion about the implementation of the cost sharing policies and provides some suggestions and recommendations for the policy makers.
