Possibilities of using MTDL substances in the treatment of Alzheimer's disease and depression

Abstract:

Práce charakterizuje podíl patofyziologických změn na rozvoji Alzheimerovy choroby a deprese. Samostatná kapitola je věnována dvěma enzymovým skupinám – cholinesterázám a monoaminooxidázám. Rozvoj Alzheimerovy choroby a deprese také ovlivňuje genetika a životospráva. V práci jsou popsány hlavní hypotézy vzniku obou onemocnění. Jejich celkový podíl na vzniku a progresi nemoci však není dodnes objasněn. Hlavním tématem rešerše je uplatnění MTDL látek v terapii Alzheimerovy choroby a deprese. Představena a kriticky zhodnocena je jejich skutečná perspektiva. Principem MTDL strategie je vývoj léčiva, které v jedné malé molekule obsahuje několik farmakoforů. MTDL látky proto terapeuticky upravují více patologických pochodů současně. Tento přístup může uspět v kauzální léčbě multifaktoriálních chorob. Hlavní pozornost je přitom kladena na Alzheimerovu chorobu, protože její současná symptomatická léčba není dostačující řešení na zpomalení prudkého nárustu počtu nemocných. Následky Alzheimerovy choroby a deprese, které fatálně ovlivňují schopnost jedince se efektivně zapojit do produktivního života, jsou pak obrovskou zátěží pro zdravotní systém a národní ekonomiky zemí na celé zeměkouli.

Abstract:

The thesis characterizes proportions of patophysiological changes in the development of Alzheimer’s disease and depression. A separate chapter is devoted to two enzyme groups – cholinesterases and monoamine oxidases. Genetics and lifestyle also influence the development of Alzheimer's disease and depression. The main hypotheses for the development of both diseases are described. However, their overall contribution to the onset and progression of the disease has not been made clear to this day. The main focus of the research is the application of MTDL substances in the treatment of Alzheimer's disease and depression. Their real perspective is presented and critically evaluated. The principle of the MTDL strategy is the development of a drug that contains several pharmacophores in one small molecule. MTDL substances therefore therapeutically modify multiple pathological processes simultaneously. This approach may succeed in the causal treatment of multifactorial diseases. The main focus is on Alzheimer's disease, because its current symptomatic treatment is not a sufficient solution to slow the rapid increase in the number of patients. The consequences of Alzheimer's disease and depression, which fatally affect an individual's ability to engage effectively in productive life, are a huge burden on the health system and national economies of countries across the globe.
