Surgical treatment of vitiligo by autologous skin cells grafting

Abstract:

Vitiligo je získané idiopatické onemocnění kůže projevující se tvorbou depigmentovaných lézí různé velikosti a tvaru s vymizením melanocytů uvnitř lézí. Kauzální léčba není možná a konzervativní terapie je u řady pacientů neúčinná. Chirurgická terapie může být účinná u některých pacientů s anamnézou neúspěšné konzervativní terapie. Všechny techniky chirurgické léčby vitiliga jsou založeny na transplantaci autologních melanocytů. V posledních letech se objevují různé modifikace chirurgické léčby od transplantace autologní kůže v plné tloušťce až po transplantaci samotných melanocytů expandovaných kultivací. Cílem této práce je chirurgická terapie vitiliga transplantací autologních nekultivovaných melanocytů s použitím tkáňového lepidla jako buněčného nosiče. Metoda spočívá v odběru dermoepidermálního štěpu, jeho následné separaci na jednotlivé buňky účinkem trypsinu a aplikaci směsi buněk ve tkáňovém lepidle na depigmentovanou kožní lézi připravenou k přijetí melanocytů ablací epidermis CO2 laserem. Metoda byla použita k léčbě vitiliga u 13 pacientů se stabilním vitiligem, pigmentace byla dosažena u 5 pacientů

Abstract:

Vitiligo is an idiopathic acquired skin disorder characterized by depigmented lesions of variable size and shape with a complete absence of melanocytes in the lesions.Causal treatment is impossible and medical treatment is ineffective with most patients. The surgical treatment can be effective with some patients with the history of ineffective conservative treatment. All techniques of surgical treatment are based on autologous melanocytes grafting. Many modifications of surgical treatment of vitiligo have apperared during the past few years, from transplantation of fullthickness skin grafts till transplantation of melanocytes expanded by cultivation. Aim of this work si surgical treatment of vitiligo by grafting of autologous noncultured melanocytes in fibrin glue which is used as a cell carrier. This method is based on grafting of dermoepidermal graft and its incubation with trypsin. The skin cells suspension which is obtained after the trypsination is then applied into a mixture with fibrin glue onto depigmented vitiliginous skin previously dermabraded by CO2 laser. This method was used for treatment at 13 patients with stabile vitiligo, pigmentation was achieved at 5 patients.
