Depression and the psychodiagnostic value of coping deficit index in the Rorschach test

Abstract:

Naše diplomová práce je pilotní studií zaměřenou na skupinu depresivních pacientů, u kterých dlouhodobá klinická zkušenost potvrzuje rezistenci na moderní farmakologickou léčbu. Cílem kvalitativní analýzy 19 případových studií je zpřesnění specifických osobnostních či širších psychologických charakteristik a souvislostí, které by mohly přispět k pochopení a vysvětlení farmakorezistence. Výsledky výzkumu jsou využity k formulaci dalších výzkumných úkolů pro rozsáhlejší studie, které by na větším vzorku pacientů mohly vést ke zpřesnění epidemiologických informací o této skupině a formulování přesných diagnostických vodítek s odpovídajícími doporučeními pro léčebné postupy. Osobnostní a další psychologické souvislosti byly zjišťované pomocí Rorschachova testu. Test byl administrován a interpretován dle standardizovaného postupu Komprehenzivního systému navrženého Johnem Exnerem. Interpretace testových obrazů se soustřeďuje na dvě proměnné, jimž jsou index deprese (DEPI) související převážně s afektivními problémy depresivního charakteru a index deficitu zvládacích schopností (CDI) poukazující spíše na sociální nezralost a těžkosti v interpersonální sféře. Výsledky výzkumu upozorňují zejména na výskyt pozitivní hodnoty CDI (10 pacientů) a pozitivní hodnoty obou indexů (další 3 pacienti). Tato zjištění naznačují, že depresivně imponující obtíže u farmakorezistentních pacientů s diagnózou nějaké depresivní poruchy mohou souviset s přítomností specifických rysů v osobnostní organizaci pacientů, které je predisponují k problémům v interpersonální oblasti a následně i k depresivní dekompenzaci.

Abstract:

Our thesis is a pilot study and focuses on the depressive patients in whom the long term clinical experience confirms resistence to pharmacological treatment. The aim of qualitative analysis of 19 case studies is to clarify and specify personality and more general psychological characteristics or implications which could contribute to explanation and understanding pharmacoresistance. Results of the survey are used for formulation of further research projects which could lead to more precise epidemiological information about this group, and to formulating specific diagnostic guidelines with corresponding reccomendations for proper treatment procedures. The Rorschach test was used as a specific method for personality and psychological implications assessment. The test was administered and interpretated according to John Exner´s Comprehensive System. The interpretation of test production focuses on the presence of two special indices, the Depression Index (DEPI) relating to affective problems connected to depression, and the Coping Deficit Index (CDI) relating to social immaturity and problems in interpersonal relationships. Results of the research draws attention especially to the presence of positive CDI (10 patients) and the presence of both CDI and DEPI (another 3 patients). These findings suggest that the depressive complaints in the patients with the diagnosis of some depressive disorder could be related to the presence of some specific features in their personality organisations which can predispose them to difficulties in their social functioning and consequently to depressive decompensations.
