Nursing Care Plan for Patient with Splenic Injury

Abstract:

Cílem bakalářské práce bylo sestavit obecný plán ošetřovatelské péče pro pacienta s poraněním sleziny v akutní péči a následně ve standardní péči s využitím NANDA International taxonomie II. Po stanovení cíle, byly odpovědi vyhledávány v elektronických databázích. Podle nalezených studií, lze tvrdit, že byly zaměřeny na diagnostiku a terapii poranění sleziny s využíváním klasifikace stupně poranění od roku 1992, tudíž nebyly nalezeny přímé důkazy k péči o pacienta s poraněním sleziny, avšak se dostává do podvědomí zdravotníků, co může poraněná slezina způsobit, a jak má být správně léčena. Dlouhodobé studie se zaměřují na pooperační komplikace, kdy nejzávažnější z nich je postsplenektomická sepse. Splenektomie pro pacienta neznamená celoživotní handicap, neboť funkci sleziny přejímá systém MFS (mononukleární fagocytární systém), který odstraňuje odumřelé erytrocyty. I když nebyly nalezeny přímé důkazy pro pacienta po poranění sleziny, byly však zjištěny informace o protokolárním řízení úrazů dle ATLS (Advence Trauma Life Support) a významném využití FAST (Focused Abdominal Sonography for Trauma) vyšetření v akutní péči pro pacienta s traumatem v břišní dutině.

Abstract:

My aim of this thesis was to compile a nursing care plan of for patients with injuries of the spleen in acute care and subsequently in the standard care with the use of NANDA International taxonomy II. After determining the objectives, have been searched for answers in electronic databases. According to the studies found, it can be argued that they were focused on the diagnosis and therapy of splenic injury with the use of the classification of the degree of injury since 1992, and therefore weren´t found direct evidence to patient care with injuries of the spleen, but is getting into the subconscious mind for health professionals, what can the injured spleen cause, and how to be properly treated. Long-term studies focus on postoperative complications, when the most serious of them is the postsplenectomic sepsis. Of splenectomy for the patient doesn´t mean a lifelong handicap, since the function of the spleen takes over the system of the MFS (mononuclear phagocytic system), which removes the dead skin cells. Although not found direct evidence for the patient after injury to the spleen, were, however, found information about Protocol control accidents according to ATLS (Advence Trauma Life Support) and significant use of FAST (Focused Abdominal Sonography for Trauma) examination in acute care for a trauma patient in the abdominal cavity.
