Elderly Patients with Depression Followed in a Cardiology Outpatient Clinic

Abstract:

Cíl: Cílem výzkumné práce bylo zjistit výskyt deprese u seniorů dispenzarizovaných ve vybrané kardiologické ambulanci. Metodika: Výzkum byl realizován kvantitativní metodou pomocí anonymního dotazníkového šetření. Pro sběr dat byly použity dva hodnotící standardizované nástroje Geriatrická škála deprese (GDS-15) a Beckova sebeposuzovací škála pro dospělé (BDI-II). Prostřednictvím dotazníku byly dále zjišťovány informace týkající se onemocnění a sociodemografické údaje. Šetření probíhalo v období květen až červenec 2018. Výzkumný soubor tvořilo celkem 133 respondentů nad 65 let, 77 mužů (58%) a 56 žen (42%). Výsledky: Při zjišťování výskytu deprese u seniorů s kardiovaskulárním onemocněním, byla deprese zaznamenána u 22,6 % respondentů s použitím škály GDS a u 27,8 % respondentů v hodnotící škále BDI-II. Dále byla nalezena statisticky významná závislost deprese se vzděláním (p = 0,015) dle škály GDS, s různými kardiologickými diagnózami (p = 0,038) dle škály GDS a funkčním vyšetřením NYHA (p = 0,016) dle škály GDS. V naší studii byl potvrzen významný korelační vztah mezi GDS a BDI-II. Ostatní výsledky věku, pohlaví a rodinného zázemí nebyly statisticky významné. Závěr: Deprese je nepříznivým nezávislým rizikovým faktore kardiovaskulárních onemocněním, proto by měla být posuzována nejen v nemocnicích, ale i u pacientů v kardiologických ambulancích.

Abstract:

Objective: The aim of the research work was to find out the occurrence of depression in elderly patients monitored in a selected cardiology surgery. Methodology: The research was carried out by a quantitative method using an anonymous questionnaire survey. For data collection were used two evaluation standardized instruments of the Geriatric Depression Range (GDS-15) and Beck´s Self-assessment Range for adults (BDI-II). Through the questionnaire was investigated information concering the disease and sociodemographic data. Research was carried out between May and July 2018. The research set consisted of 133 respondents over 65 years old, 77 men (58%) and 56 women (42%). Results: In detecting depression at elderly patients with cardiovascular disease, the depression was detected at 22,6% of respondents using the GDS scale and at 27,8% of respondents using BDI-II scale. In addition, there was found statistically significant education-related depression (p = 0,015) according to the GDS scale, various cardiologic diagnoses depression (p = 0,038) according to the GDS scale and with functional examination NYHA (p = 0,016) according to the GDS. In our study, a significant correlation relationship between GDS and BDI-II was confirmed. Other results of age, gender and family background were not statistically significant. Conclusion: Depression is an unfavourable independent risk factor for cardiovascular disease, therefore it should be assessed not only in hospitals but also at patients in cardiology surgeries.
