Abstract:

Cíl: U polytraumatických pacientů je přetrvávající katabolismus jednou z příčin opožděného hojení. Dlouhodobě existuje snaha katabolismus zmírnit. Cílem studie bylo ověřit efekt časného podání kombinace anabolického steroidu s vitaminem D na potlačení katabolismu, a to hodnocením aktivity kostního metabolismu u polytraumatizovaných nemocných. Metody: V prospektivní studii byli pacienti se závažným polytraumatem rozděleni do dvou skupin - intervenované (u pacientů byl podáván anabolický steroid nandrolon dekanoát a vitamin D) a kontrolní skupiny (bez podávání léků). U všech pacientů byly po dobu 70 dnů laboratorně monitorovány markery kostního metabolismu a hladiny pohlavních hormonů. Následně byly skupiny porovnány. Výsledky: Nebyl prokázán pozitivní efekt intervence na parametry kostního metabolismu. U intervenované skupiny byla 70. den identifikována vyšší hladina vitaminu D (P ? 0,001) a od 7. dne u mužů intervenované skupiny signifikantně nižší hladina testosteronu a volného testosteronu. Závěr: Během sledovaného období nebyl prokázán pozitivní efekt aplikace anabolického steroidu a vitaminu D na kostní metabolismus u polytraumatizovaných pacientů. Potvrdil se ale význam zapojení kostního metabolismu v akutní stresové reakci.

Abstract:

Aim: Persistent catabolism is one of the main reasons for delayed healing in polytrauma patients. The effort shall be oriented toward decreasing the amount of catabolism. The aim of this study was to investigate the possible positive effects of administering an anabolic steroid with vitamin D on bone metabolism in polytrauma patients. Methods: In this prospective study, patients with serious polytrauma were divided into the control and intervention groups. Patients in the intervention group received an anabolic steroid nandrolone decanoate and vitamin D in addition to the standard treatment. In all patients, laboratory markers of bone metabolism, parathyroid and sex hormones were observed for 70 days and results compared between the groups. Results: The expected positive effect of the intervention on the bone metabolism parameters was not proved. The only significant differences were the expected increase in vitamin D levels on Day 70 (P ? 0.001) in patients from the intervention group along with a significant decrease of total and free testosterone in male patients within the control group. Conclusions: Over the observed period, no positive effect of the administration of the anabolic steroid nandrolone decanoate in combination with vitamin D on the bone metabolism in polytrauma patients was observed. But there has been confirmed the importance of the bone metabolism in acute stress reaction.
