Theory of mind in patients with psychotic disorder and its realtionship to cognitive functions

Abstract:

Recent studies revealed a deficit of theory of mind in patients suffering from psychotic disorders. This deficit could explain some of the psychotic symptoms. But because psychotic disorders are often accompanied by cognitive deficit we should know the relationship between both of them. The aim of this study is therefore to find out what is the relationship between theory of mind and cognitive function in psychotic disorder. We tested 32 men with psychotic disorder (21 with schizophrenia and 11 with schizoaffective disorder) using neuropsychological methods and the stories evaluating theory of mind of first and second order and understanding of metaphors. We found that the patients with schizophrenia were unable to infer the mental states of characters presented in the stories. The results showed no significant difference between patients with schizophrenia and patients suffering from psychoaffective disorder. Neither we found any differences in ability of mentalizing between patients during remission and patients during manifest episode or between first hospitalized patients and chronic ones. Our results revealed a significant positive relationship between theory of mind and cognitive functions (specifically executive functions, shor-term verbal memory and delayed recall and attention). We also found a strong positive relationship between theory of mind and years of study.

Abstract:

Nedávne výskumy ukazujú, že pacienti so psychotickým ochorením majú deficit v teórii mysle, čo by mohlo vysvetľovať niektoré z príznakov ochorenia. Zároveň je ale známe, že u pacientov v dôsledku ochorenia dochádza k poškodeniu kognitívnych funkcií. Preto sa naskytá otázka vzťahu medzi stavom kognitívnych funkcií a teóriou mysle u týchto pacientov. Zistiť aký je vzťah medzi teóriou mysle a kognitívnymi funkciami bolo hlavným cieľom tejto práce. Pomocou neuropsychologických testov a príbehov prvého a druhého rádu teórie mysle a porozumenia metafór sme testovali 32 mužov s psychotickým ochorením (21 s diagnózou schizofrénie a 11 s diagnózou schizoafektívnej poruchy). Zistili sme, že neboli schopní prisudzovať aktérom v príbehoch mentálne stavy, pričom sa nenašiel významný rozdiel medzi pacientmi s diagnózou schizofrénie a schizoafektívnej poruchy. Rovnako sme nenašli rozdiel medzi výkonom v teórii mysle u pacientov počas remisie a tými v akútnej fázy ochorenia, ani medzi pacientmi prvý krát a opakovane hospitalizovanými. Naše výsledky ale ukázali významný pozitívny vzťah medzi teóriou mysle a kognitívnymi funkciami (konkrétne exekutívnymi funkciami, krátkodobou a dlhodobou verbálnou pamäťou a pozornosťou). Rovnako sme našli významný pozitívny vzťah medzi teóriou mysle a počtom rokov štúdia.
