Physiotherapy after fracture of the proximal humerus

Abstract:

Cíl práce: Cílem práce je přispět k řešení problematiky fyzioterapie u zlomenin v oblasti proximální části humeru a prostřednictvím fyzioterapeutické intervence pomoci k časnému návratu pacienta do plnohodnotného života. Metodika: Jedná se o kazuistiku 54letého pacienta s diagnózou zlomenina proximálního humeru. S pacientem jsem pracoval v období od 20.9.2014 do 25.11.2014. Během toho období jsem také provedl kontrolní vyšetření ve 3. a 8. týdnu. Během vstupního kineziologického rozboru jsem posoudil aktuální stav měkkých tkání včetně jizev, svalové síly v oblasti ramenního kloub a rozsahy pohybu. Změřil jsem obvody a délky horních končetin. Sledoval jsem stereotyp pohybů v ramenním kloubu, chůzi. Na základě výsledků jsem stanovil krátkodobý fyzioterapeutický plán. Výsledky: Objevené patologie byly blokády v glenohumerálním, akromioklavikulární a skapulotorakální skloubení, snížený rozsah pohybu v pravém ramenním kloubu, svalové dysbalance, špatný stereotyp abdukce v pravém ramenním kloubu a patologie měkkých tkání. Na základě výsledků vyšetření jsem stanovil terapii v podobě: kryoterapie, míčkové facilitace, kineziotapingu, polohování, techniky měkkých tkání, manuální centrace dle Čápové, PIR, PFS, izometrického cvičení a cvičení s pomůckami (tyč, overball), PNF a DNS. V průběhu terapie došlo ke zlepšení funkčního ROM pravého ramenního kloubu, posílení svalových skupin v oblasti pravého ramenního kloubu, snížení otoku PHK, zlepšení stereotypu abdukce a odstranění kloubních blokád. Závěr: Působením intenzivní fyzioterapeutické intervence během ambulantní léčby a vlivem edukace k autoterapii v domácím prostředí bylo dosaženo zlepšení ROM pravého ramenního kloubu, svalové síly svalových skupin v oblasti pravého ramenního kloubu, odstranění kloubních blokád, zlepšení stereotypu abdukce v pravém ramenním kloubu, snížení otoku PHK. Během terapie nedošlo k žádné komplikaci. Ačkoliv nedošlo k úplnému obnovení rozsahů pohybu zejména do rotací a flexe v pravém ramenním kloubu, je vidět účinek fyzioterapie a autoterapie. Pacient se může vrátit do své profese a k plnohodnotnému vedení života.

Abstract:

Objective: to contribute to solving problems of physiotherapy for fractures of the proximal part of the humerus and through physiotherapy interventions to help patients early return to a full life. Methods: This bachelor´s thesis describes a case of 54year man with diagnosis of proximal humerus fracture. I worked with the patient in the period from 20. september 2014 to 24 november 2014. During that period, I also carried out control examinations in the 3rd and 8th week During the entire analysis from the begining I have reviewed the current status of soft tissues including scarring, muscle strength in the shoulder joint and range of motion, I measured the girth and length of the upper limbs. I watched stereotype movements in the shoulder joint, walking. Based on the results I set short-term physiotherapy plan. Results: pathology were discovered blockage in the glenohumeral, acromioclavicular and scapulothorakale joint, decreased range of motion at the shoulder joint, muscle disbalance, bad stereotype abduction at the shoulder joint and soft tissue pathology. Based on the results of the examination I established therapy in the form of: cryotherapy, ball facilitation, kineziotaping, positioning, soft tissue techniques, manual centration of shoulder joint, PIR, PFS, isometric exercises and exercises with tools (rod, overball) PNF and DNS. During therapy improved functional ROM of the right shoulder, strengthening the muscle groups in the area of the right shoulder, reduce swelling PHK, improvement stereotype abduction and elimination of joint blocks. Conclusion: effects of intensive physiotherapy intervention during outpatient treatment and education due to self-therapy in the home setting to achieve improvements in ROM of the right shoulder, muscle strength muscle groups in the area of the right shoulder, eliminating blocked joints, improve stereotype abduction at the shoulder joint, reduce swelling PHK. During treatment, there were no complications, although not completely restore the range of motion in particular in rotation and flexion in the right shoulder joint, it is to see the effect of physiotherapy and autotherapy. The patient can return to his profession and to lead a fulfilling life.
