Physioterapy after total hip replacement

Abstract:

Cíl: Hlavním cílem mé bakalářské práce je informovat čtenáře o problematice totální endoprotézy kyčelního kloubu získané studiem odborné literatury. Dále také poskytnout ucelený obraz předoperační a pooperační péče u totální endoprotézy kyčelního kloubu. Metodika: U obou pacientů, s kterými jsem pracovala, byla diagnostikována osteoartróza kyčelního kloubu, následně byly tito dva pacienti indikováni k totální endoprotéze kyčelního kloubu. U paní M. H. jsem nastavila dobu rehabilitace na 2 měsíce, u pana L.K. na 15 dní. Stanovila jsem kontrolní vyšetření se zaměřením na sílu stabilizátorů pánve a to 5., 10., 15., den po operaci a 2 měsíce od implantace totální náhrady. V pooperačním období jsem volila fyzioterapeutické postupy dle Lewita - techniky měkkých tkání, využití postizometrické relaxace pro uvolnění hypertonických svalů a postfacilitačního strečinku k protažení svalů zkrácených. Dále jsem využila RS pro zvětšení svalové síly a stabilizaci kyčelního kloubu. Mezi pomůcky jsem zařadila hlavně overball a Thera-band s přihlédnutím k Brügger konceptu, kterými jsem se snažila zvětšit rozsah pohybu a také svalovou sílu. V neposlední řade byl využit kinesiotape na jizvu a také facilitací tape abduktorů kyčelního kloubu. Výsledky: Výstupní kineziologický rozbor ukázal zlepšení a to hned v několika parametrech: zvětšení rozsahů pohybů operované dolní končetiny do flexe, extenze, abdukce, vnitřní rotace, nárůst svalové síly operované končetiny - zvětšení v průměru o 1 stupeň, odstranění otoků, patologické bariéry v oblasti inquinální fascie, správné stereotypy provádění pohybů do extenze a abdukce kyčelních kloubů bilaterálně, snížení stupně zkrácení svalových skupin ve všech třech zjištěných případech, správný stereotyp chůze.

Abstract:

Purpose: Purpose of this work is to get new theoretical knowledge in the issue, inform the readers and throught short-term rehabilitation plan to improve the mobility of a patient who underwent surgery total arthroplasty of the hip joint. Methodology: In both patients, with whom I worked, was diagnosed osteoarthritis of the hip joint, followed by the two patients were triaged to total hip arthroplasty. For Mrs. M.H. I set period of rehabilitation at 2 months of Mr. L.K. rehabilitation has been set for 15 days. I set-up examination, focusing on the strength of pelvic stabilizers and the 5th, 10th, 15th day after surgery and 2 months after implantation of total hip replacement. In the postoperative period, I chose physiotherapy by Lewit- soft tissue techniques, use of a postisometric relaxation for release of hypertensive muscles and post facilitation stretching for muscles that are tight. I also used RS to increase muscle strenght and stabilization of a hip joint. Among physiotherapy equipment I mainly used are overball and Theraband according to Bruggers concept with those I¨ve tried to increase a range of movement and muscle strenght. At last but not least Ive used kinesiotape on a scar and to facilitace hip abductors. Results: An output assesment showed an improvement in many criteria: an increase in range of movement on the operated leg in flexion, extenion, abduction and inner rotation, a gain in a muscle strenght on the operated leg- an increase in average of one degree,a removal of a swelling, patological barrier of an inguinal fascia, correct movement stereotypes of hip extension and abduction bilaterally, a lowered level of a shortened muscles in all three assesed cases, correct gait movement.
