Liquid biopsies in multiple myeloma

Abstract:

Současným standardem v diagnostice a monitoringu mnohočetného myelomu zůstává i přes veškerý pokrok v posledních letech biopsie kostní dřeně. Tato technika je bohužel invazivní a heterogenita vzorků je omezena pouze na odběrovou oblast. Díky novým terapeutickým možnostem dosahují pacienti s mnohočetným myelomem v současnosti hlubších odpovědí na léčbu než kdy předtím, a pro jejich léčbu se proto stává zcela zásadním sledování minimální residuální choroby. Nový, minimálně invazivní přístup tzv. „tekuté biopsie“ využívá k analýze vzorky tělních tekutin. Tento přístup tedy není limitován pouze na jednu oblast a získané informace jsou tudíž komplexnější než u klasické biopsie. Tato práce se zaměřuje na využití tekutých biopsií k analýze minimální residuální choroby s využitím cell-free DNA z periferní krve pacientů mnohočetného myelomu. Analýza vzorků od 45 pacientů ukazuje, že hladiny tumor-specifické cell-free DNA signifikantně korelují s indukční léčbou (p=0,004) a taky s autologní transplantací kmenových buněk (p=0,012). Navíc podle analýzy fragmentů cell-free DNA, souvisí samotná délka fragmentů s odpovědí na léčbu. Naše výsledky tedy podporují možnost využití cell-free DNA jako prognostického markeru u pacientů s mnohočetným myelomem.

Abstract:

Despite of progress made in the field of multiple myeloma in the last decade, site-limited bone marrow biopsy is still a gold standard used in its diagnostics and monitoring. As patients currently achieve deeper responses than ever before, an assessment of minimal residual disease becomes paramount for their further treatment. Liquid biopsies represent a minimally invasive and more comprehensive technique that is not site-limited and provides more complex information. This thesis aimed to explore the use of liquid biopsy for monitoring of multiple myeloma patients via cell-free DNA isolated from peripheral blood. Data from 45 patients show that tumor-specific cell-free DNA levels significantly correlate with induction therapy (p=0.004) and autologous stem cell transplantation (p=0.012). Additionally, the analysis of cell-free DNA fragments indicates that the length of fragments is associated with treatment response of patients. These results support the concept of tumor-specific cell-free DNA as a prognostic marker for multiple myeloma patients.
