Diplopia with convergence insufficiency in a patient without strabismus

Abstract:

Diplopie a poruchy zrakových funkcí jsou v posledních letech problémem, který se prolíná napříč celou populací. S vývojem přístrojového vybavení, dokonalejší diagnostiky a díky moderním zobrazovacím metodám, máme možnost lépe a účinně cílit terapii u daného jedince. Během stanovení ortoptického statusu je využívána řada přístrojů a principů optického zobrazení, a to jak u dětských, tak dospělých pacientů. Problematika práce s digitálními zařízeními je v současné době velmi aktuální. Proto je cílem práce zjistit, jaké symptomy se u pacientů objevují a jaké přístrojové vybavení lze využít při jejich diagnostice. Do retrospektivní studie bylo zařazeno 51 probandů z řad zaměstnanců počítačových firem, kteří tráví většinu své pracovní doby s elektronickým zařízením. Věkové rozmezí probandů bylo 26-36 let, tedy šlo o nepresbyopický věk. Dlouhodobé výsledky byly hodnoceny na základě pravidelně se opakujícího kompletního ortoptického vyšetření. V posledních několika letech se objevuje více dospělých pacientů s astenopickými potížemi nebo diplopií. Etiologii těchto příčin nacházíme často v malé nebo nedostatečné velikosti šířky fúze. Sběr dat probíhal v období let 2015–2017. U asymptomatické skupiny nevznikl statisticky významný rozdíl ve velikosti fúzních rezerv před a po jejich cvičení. U symptomatické skupiny vznikl statisticky významný rozdíl ve velikosti fúzních rezerv před a po cvičení. U velikostí pozitivní šířky fúze došlo k jejímu navýšení, naopak u negativní šířky fúze ke snížení hodnot. Metody sloužící k diagnostice fúzní šířky na synoptoforu a pomocí prizmat v prostoru spolu vzájemně korelují. Bylo zjištěno, že přítomnost symptomů je závislá na hodnotě pozitivní fúzní šířky. Hranice této hodnoty byla stanovena na 10 cm/m a bylo potvrzeno, že u hodnot nižších nebo rovno 10 cm/m se vyskytovaly symptomy diplopie či pocitu očního dyskomfortu při práci do blízka. U hodnot vyšších než 10 cm/m se tyto symptomy nevyskytovaly. Subjektivní příznaky pacientů statisticky významně závisí na velikosti pozitivních fúzních rezerv. U asymptomatické skupiny nedošlo po cvičení ke statisticky významnému snížení insuficience konvergence diagnostikované pomocí přístroje Maddox wing. Naopak u symptomatické skupiny došlo po cvičení ke statisticky významnému snížení insuficience konvergence. Synoptofor, prizmata a Maddox wing jednoznačně patří mezi základní diagnostiky, které vedou ke zjištění symptomů. Naše studie prokázala důležitost měření heteroforií do blízka a velikosti fúzních rezerv. Potvrdila, že řada symptomů, jako je např. diplopie do blízka, astenopické potíže či bolesti hlavy, může souviset s nedostatečnou velikostí pozitivní fúzní rezervy, jejíž velikost ovlivňuje právě vznik těchto symptomů. Dále jsme potvrdili, že ortopticko-pleoptická cvičení patří mezi nejdůležitější pro léčbu insuficience konvergence a nedostatečné fúzní rezervy.

Abstract:

In recent years diplopia and visual impairments have become an issue that affects people across the entire population. Due to the advancement in development of instrumentation, improvement in diagnostics and modern imaging methods, we are able to customise therapy for any given individual in a better and more efficient way. The orthoptic status is determined, both in paediatric and adult patients, utilizing a number of devices and optical imaging principles. At present, the issues of working with digital devices are highly relevant. The goal of this thesis therefore is to find out which symptoms occur in patients and what kind of instrumentation can be used in their diagnosis. The retrospective study included 51 research subjects, all of whom were employees in IT companies who spend most of their working time with digital devices. The research subjects’ age ranged between 26 and 36 years, i.e. non-presbyopic age. The long term results were evaluated based on periodically repeated complete orthoptic examinations. In the recent years there have been more adult patients presenting with asthenopia or diplopia. From an aetiological standpoint, these issues are often caused by an insufficient size of the fusional amplitude. The data were collected between the years 2015 and 2017. There was no statistically significant difference in the size of fusion reserves before and after exercises in the asymptomatic group. In the symptomatic group, there was a statistically significant difference in the size of fusion reserves before and after exercises. The size of the positive fusional amplitude increased, while the negative fusional amplitude decreased. The methods used to diagnose the fusional amplitude on the synoptophore and using prisms in free space correlate with each other. The presence of symptoms was found to be dependent on the value of the positive fusional amplitude. The limit of this value was set at 10 cm/m and it was confirmed that at values lower than or equal to 10 cm/m there were symptoms of diplopia or a presence of subjective eye discomfort while performing near distance tasks. At values higher than 10 cm/m these symptoms did not occur. The subjective symptoms of patients statistically significantly depend on the size of their positive fusional amplitude. In the asymptomatic group, there was no statistically significant reduction in convergence insufficiency diagnosed with the Maddox wing after exercises. In contrast, in the symptomatic group there was a statistically significant reduction in convergence insufficiency after exercises. Synoptophore, prisms and Maddox wing clearly belong to the basic diagnostic methods that lead to the detection of symptoms. Our study demonstrated the importance of measuring the presence of near heterophoria and the size of fusional reserve. The study has confirmed that many symptoms, such as diplopia at near, asthenopia or headaches may be related to the insufficient size of the positive fusional reserves, the size of which affects the onset of these symptoms. We further confirmed that orthoptic-pleoptic exercises are among the most important for the treatment of convergence insufficiency and insufficient fusional amplitude.
