Changes in Target Volume during Radiotherapy

Abstract:

Tato bakalářská práce je zaměřena na změnu cílových objemů během radioterapie. Samotná práce je rozdělena na několik částí. V první části jsou popsány cílové objemy, které jsou užívány v radioterapii. Metody lokalizace těchto objemů a rozdělení a princip simulátorů. V další kapitole jsou popsány samotné změny cílových objemů a možnost jejich sledování za pomoci IGRT. Ve třetí kapitole se zabývám technikami, u kterých je největší problém se změnou cílových objemů. Je to technika IMRT a SIB. Je zde také probráno stanovení PTV a dávky záření. Čtvrtá kapitola je věnována nádorům hlavy a krku, jejich diagnostice a léčbě. Nádory hlavy a krku jsou zde rozděleny do dvou skupin. První skupina jsou nádory, u kterých je přeplánování velice běžné. Druhá skupina jsou nádory, kde je přeplánování výjimečnou záležitostí. Následující kapitola je zaměřena na adaptivní radioterapii. Poslední část této práce je studie, která se zabývá změnou cílových objemů během radioterapie u nádorů krku. Jsou zde srovnávány nádory při první, desáté a devatenácté frakci.

Abstract:

This bachelor thesis is focused to changes in target volumes during radiotherapy. It is divided into several parts. The first part describes target volumes used in radiotherapy. Methods of lokalization such volumes, division and the princip of simulator. The next chapter describes in target volumes themselves and the posibility of monitoring them using the IGRT. The third chapter looks into the techniques in which the biggest problem lays in the change in target volumes. These are the IMRT and SIB. It also deals with setting the PTV and the dose of radiation. The fourth chapter is dedicated to head and neck tumours, their diagnosis and treatment. There is a set example of two groups of the head and neck tumours. The first one are tumors in which replanning is very common. The second group are tumors with the replanning to be exceptional issue. The following chapter aims at the adaptive radiotherapy. The last part of this thesis includes a study, which focuses on the change in target volumes during a radiotherapy of neck tumours. Tumors are compared in the first, tenth and the nineteenth fractions.
