The analysis of the trainees’ reflecting-responding capacity in Training in psychotherapy integration

Abstract:

Cílem této popisné kvalitativní studie je analýza reflektivně-responzivní kapacity (RK) frekventantů Výcviku integrace v psychoterapii (VIP). Představíme model prožívání frekventantů před vstupem do výcviku, rozhodování o vstupu do výcviku a rozvoj RK v průběhu prvních 3 let výcviku. V teoretické části se věnujeme oblasti vzdělávání v psychoterapeutických výcvicích v České republice i v zahraničí. Vzdělávání v psychoterapeutickém výcviku je uvedeno do kontextu teorie sociálního poznávání, sociálních skupin a systémů. Představeny jsou vlivy na rozhodování o výběru teoretické orientace i výcviku v psychoterapii i vybrané vývojové modely napříč kariérou psychoterapeuta. Zdroje pro vývoj v psychoterapeutickém výcviku představujeme souhrnně jako tzv. „výcvikový potenciál“. Práce zahrnuje téma přenosu zkušeností z výcviku mimo výcvikovou skupinu či komunitu a naopak a pojetí psychoterapeutického výcviku jako tzv. „autopoietického“ („sebeorganizujícího se“) sociálního systému. V empirické části je hlavní výzkumnou metodou zakotvená teorie (ZT) doplněná prvky konsenzuálního a narativního výzkumu. V průběhu výzkumu byla na míru dat vytvořena tzv. Sdílená Metoda Analýzy a Obohacování Dat (SMAOD). Zkoumaný soubor tvořilo 26 frekventantů výcviku, z toho 21 žen a 5 mužů (v průměrném věku 26,7 roku na začátku výzkumu) a výzkumnice samotná (ve věku 25 let na začátku výzkumu). Data sestávají z motivačních dopisů psaných před vstupem do výcviku, rozhovorů a dotazníků vytvořených před začátkem výcviku, závěrečných prací a dotazníků psaných frekventanty výcviku na konci každého z prvních 3 let výcviku. Další data zahrnují 3 rozhovory s frekventanty, kteří byli do výcviku přijati, ale rozhodli se nenastoupit. Poslední zdroje, určené pro triangulaci dat, tvoří 5 analytických rozhovorů výzkumného týmu a výcvikový deník autorky studie z výcviku SUR. Výsledky tvoří tři navazující sekce: (1) prožívání frekventantů před vstupem do VIP; (2) proces nabývání jistoty ke vstupu do VIP; (3) vývoj tzv. reflexivní, resp. reflektivně-responzivní kapacity (RK) a integrační kapacity (IK) v průběhu prvních 3 let výcviku. V diskuzi jsme se zaměřili na porovnání stávajících modelů rozvoje frekventantů v psychoterapeutických výcvicích se zkušenostmi frekventantů VIP. Dále na porovnání RK se složkami obecného konceptu „inteligence“ podle struktury Wechslerova inteligenčního testu (WAIS-III). Výsledný model je diskutován v kontextu vybraných psychoterapeutických přístupů v psychoterapii. Diskutována je intersubjektivita při sociálním poznávání a učení, např. dle konceptu „zóny proximálního vývoje“ L. S. Vygotského či konceptu „ekvilibria“ dle J. Piageta. Cílem práce je poskytnout zpětnou vazbu frekventantům výcviků i lektorům a podpořit pozitivní změny v oblasti vzdělávání ve výcvicích v psychoterapii.

Abstract:

The research goal of this descriptive qualitative study was the analysis of the trainees‘ reflective-responding capacity (RC) in Training in psychotherapy integration (TPI). The model of trainees‘ experience before entering TPI, the process of making the decision to enter TPI and the development of RC during the first 3 years of the training were introduced. Education in psychotherapy training in the Czech Republic and in foreign countries were introduced in the theoretical section. Education in psychotherapy training was put into the context of the theory of social knowledge, social groups and systems. Influences on the choice of psychotherapeutic orientation and training were introduced, along with various models of how therapists develop throughout their careers. Development resources in psychotherapy training were comprehensibly labelled “training potential”. The study also covers the issue of transmitting experiences gained from training to outside of the training group or community (and vice versa) as well as understanding training as an autopoietic (self-organizing) social system. In the empirical section, the main research method was defined as grounded theory matched by consensual qualitative and narrative research. In the course of the research, a unique Shared Analysis Method (SAM) tailored to the data analyzed was created. The sample consisted of 26 trainees, 21 women and 5 men (whose mean age was 26.7 years at the beginning of the research) and the researcher herself (25 years of age at the beginning of the research). The data consist of motivational letters, interviews and questionnaires created before the start of the TPI and final reports and questionnaires completed by trainees at the end of each of the first three years of TPI. Additional data were comprised of 3 interviews with trainees enrolled in TPI who decided not to enter the training. The final data resources used with the aim of data triangulation consist of 5 analytic interviews of the researchers themselves and the self-reflective diary of the author of this study from her psychotherapy training (SUR). The results were divided into 3 sections: (1) trainees’ experiences before the start of TPI; (2) gaining certainty to enter TPI; and (3) description of the trainees’ reflective-responding capacity (RC) and integration capacity (IC) during the first 3 years of TPI. The discussion focuses on comparing the contemporary models of development in psychotherapy training with the experience of TPI trainees. The RC model was compared with the components of intelligence in accord with the structure of the Wechsler intelligence test (WAIS-III). The resulting model was put into the context of selected psychotherapeutic approaches. Inter-subjectivity in social knowledge and learning was discussed according to concepts such as the “zone of proximal development” according to L. S. Vygotsky or “equilibration” according to Piaget. The aim of this study was to provide feedback to psychotherapy trainees and trainers and to facilitate positive changes in psychotherapy training education.
