Role of statistical data analysis in organized programmes for secondary prevention of cancer

Abstract:

Sekundární prevence (screening) zhoubných nádorů prsu, tlustého střeva a konečníku a děložního hrdla je prokazatelně účinná při snižování úmrtnosti na tato závažná onemocnění. Pro garanci jejich bezpečnosti, účinnosti a nákladové efektivity je nezbytná jejich implementace ve formě organizovaných programů. Nezbytnou součástí organizovaných programů je systém informační podpory, který zahrnuje monitoring kvality screeningového procesu a zároveň epidemiologický monitoring populačního dopadu těchto programů. Tato diplomová práce pojednává o implementaci systému informační podpory v programech screeningu zhoubných nádorů v České republice. Úvod práce shrnuje důkazy o účinnosti jednotlivých programů a mezinárodní doporučení, dále pak roli statistického zpracování dat a podíl autora diplomové práce na realizaci informační podpory českých organizovaných screeningových programů. Je rozpracována problematika statistického hodnocení screeningového procesu v centrech mamografického screeningu. Analýza indikátorů kvality je provedena prostřednictvím běžných standardizačních metod a stochastických modelů (marginálních a podmíněných modelů), které umožňují statistickou validaci a rovněž odhalení vlivu vysvětlujících proměnných na hodnoty indikátorů kvality. Navrhnuté metody jsou dostatečně obecné pro využití v rámci dalších screeningových programů i v jiných odvětvích zdravotní péče. Práce dále popisuje implementaci jednotlivých organizovaných screeningových programů v České republice a jejich výsledky. Nezbytnou součástí informační podpory je rovněž monitoring populačního dopadu programů. Je popsána metodika populačního modelování incidence zhoubných nádorů a její aplikace pro predikci počtu pravděpodobně léčených pacientů v roce 2009. Práce rovněž obsahuje metodiku a výpočet populačního přežití onkologických pacientů, které patří mezi epidemiologické indikátory dopadu programu. Závěrečná část obsahuje analytické podklady pro návrh projektu prevence karcinomu plic. Uvedená analýza demonstruje využití dostupných epidemiologických dat a syntézu informací z odborné literatury při návrhu potenciálně účinných preventivních programů.

Abstract:

Secondary prevention (screening) is an effective way to decrease mortality rates from breast, colorectal, and cervical cancers. To guarantee its effectiveness, safety and cost-effectiveness, these preventive efforts need to be implemented as organized programmes. A system for information support is an essential element of organized programmes: such system comprises performance monitoring of the screening process and epidemiological monitoring of impact of these programmes. This diploma thesis describes how the system for information support has been implemented in Czech organized cancer screening programmes. The introduction summarizes evidence for the efficacy and effectiveness of programmes, as well as international recommendations. It also explains the role of statistical data analysis and the author’s involvement in the realization of information support for Czech cancer screening programmes. Statistical methods for monitoring of screening process at mammography screening centres are discussed. Performance indicators are analysed and compared using usual standardization techniques and stochastic models (marginal and conditional models), which enable their statistical validation and reveal the influence of different predictors on values of performance indicators. The proposed methods are universal enough to be employed in other screening programmes and even in different areas of health care. The thesis also describes the implementation of organized screening programmes in the Czech Republic and their results. The monitoring of population impact is a vital part of information support system. Methods for population cancer incidence modelling are described and used to predict numbers of patients potentially treated in year 2009. This thesis also deals with methods for the estimation of population survival of cancer patients and provides recent results. Population survival is one of epidemiological indicators, which assess the impact of a screening programme. The last part of the thesis describes information support for a proposed project on lung cancer prevention. It demonstrates how the analysis of epidemiological data and evidence synthesis can be helpful to design potentially effective prevention programmes.
