Early Mobilization of Patients with Obesity after Stroke

Abstract:

Tato diplomová práce se v obecné části zabývá teorií a komplexní rehabilitací cévní mozkové příhody, věnuje se také problematice obezity. Cílem této práce bylo zhodnotit efektivitu časné vertikalizace u osob po cévní mozkové příhodě s obezitou pomocí škály pro hodnocení stavu po cévní mozkové příhodě (NIHSS - National Institute of Health Stroke Scale), testu funkční nezávislosti (FIM – Functional Independence Measure) a testu hodnocení stability sedu (FIST – Function in Sitting Test). Cílem také bylo zhodnotit vztahy mezi výsledky výstupních testů a výsledky vstupních testů, věkem pacientů, délkou rehabilitace a hodnotou Body Mass Index (BMI) u pacientů a ověřit platnost těchto zmíněných testů. U pacientů s diagnózou cévní mozkové příhody a s obezitou došlo ke statisticky významnému zlepšení hemiparetického postiţení, funkční nezávislosti i stability sedu, objektivizované výše uvedenými testy. Výsledky výstupních testů jsou závislé na hodnotách testů vstupních a naopak nejsou závislé na věku a BMI pacientů, ani na délce rehabilitace. Testy NIHSS, FIM a FIST jsou tedy platnými testy pro hodnocení stavu pacientů po cévní mozkové příhodě s obezitou.

Abstract:

In the theoretical part, the thesis deals with the theory and complex rehabilitation of stroke, as well as the topic of obesity. The thesis aims to evaluate the effectivity of early verticalisation for people with obesity after stroke by means of NIHSS – National Institute of Health Stroke Scale, FIM – Functional Independence Measure and FIST – Function in Sitting Test. It also aims to evaluate correlations between results of the initial test and of the output, the age of the patients, the length of the rehabilitation and Body Mass Index value. It further aims to verify the validity of those tests. There has been a statistically significant improvement in terms of hemiparesis, functional independence and sitting stability at the patients with obesity after stroke, objectively proven by above-mentioned tests. Results of the output are dependent on the values of the initial tests. On the contrary, it does not depend on either the age or BMI of the patients or the length of the rehabilitation. Tests NIHSS, FIM and FIST are therefore valid tests for evaluation of the patient’s condition after stroke and with obesity.
