Variability, ontogeny and phylogeny of pronator teres muscle and their clinical significance

Abstract:

Úvod. Variabilita musculus pronator teres může hrát roli při vzniku úžinových syndromů nervus medianus. Pozornost byla věnována hlavně vztahu musculus pronator teres a processus supracondylaris humeri, který může sloužit jako variabilní odstupové místo tohoto svalu. Sledováno bylo i uspořádání těchto útvarů u lidských embryí a plodů a také analogických struktur u vybraných druhů savců. Metody. Frekvence výskytu processus supracondylaris byla sledována u pěti historických osteologických souborů a rentgenologicky na vzorku recentní české populace. Variabilita musculus pronator teres byla sledována na 68 anatomických preparátech horních končetin. V komparativní části byly preparovány končetiny šesti vybraných druhů zvířat. Uspořádání sledovaných struktur bylo zkoumáno klasickou pitvou u třech lidských fétů a dále bylo použito skenování pomocí microCT u dvou embryí. Výsledky. V souboru z Vedrovic se processus supracondylaris vyskytoval s frekvencí 2,1%, vzhledem k počtu jedinců a 1,1%, vzhledem k počtu kostí. V souboru z Hoštic se processus supracondylaris vyskytoval s frekvencí 2,1%, vzhledem k počtu jedinců a 1,1%, vzhledem k počtu kostí. V souboru z Brna – Maloměřic se processus supracondylaris vyskytoval s frekvencí 5,1%, vzhledem k počtu jedinců a 2,6%, vzhledem k počtu kostí. V souboru z Pohanska se processus supracondylaris vyskytoval s frekvencí 1,8%, vzhledem k počtu jedinců a 0,9%, vzhledem k počtu kostí. V osteologické sbírce se processus supracondylaris vyskytoval s frekvencí 2,1%, vzhledem k počtu kostí. Radiologická studie prokázala výskyt processus supracondylaris v 0,6%. Na studovaných horních končetinách dospělých jedinců začínala humerální hlava musculus pronator teres vždy od epicondylus medialis humeri a v 70,6% případů i od septum intermusculare brachii mediale. V 1,5% byla humerální hlava zdvojená. Ulnární hlava absentovala v 4,4% případů; v 66,2% případů byla svalová, ve 20% pak šlašitá a v 13,8% smíšená. Průměrná výška odstupu svalu z pažní kosti v oblasti mediálního epikondylu se pohybovala okolo 20 mm. Nervus medianus procházel mezi oběma hlavami musculus pronator teres v 85,3%, ulnární hlavu prorážel v 5,9% a v 2,9% probíhal pod oběma hlavami. V jednom případě spolu s nervus medianus probíhaly mezi oběma hlavami musculus pronator teres vasa ulnaria. Processus supracondylaris ani Struthersovo ligamentum nebyly na materiálu zaznamenány. V komparativní části proběhla srovnávací pitva šesti druhů savců, z nichž čtyři (kotul, lemur, kočka a křečík), měli vytvořeno foramen supracondylare. Musculus pronator teres začínal u lemura a křečíka jak z mediálního epikondylu, tak z kostěného oblouku přepažující foramen supracondylare. U kočky, křečka zlatého, kotula a psa pouze z mediálního epikondylu. U kotula začínal musculus pronator teres dvěma hlavami, podobně jako u člověka. U tří studovaných lidských plodů byla zjištěna stejná úprava musculus pronator teres včetně vztahu k nervus medianus jako u dospělého jedince. Přítomnost processus supracondylaris nebyla zaznamenána. U dvou lidských embryí nebyla taktéž zaznamenána přítomnost processus supracondylaris. Závěr. Výskyt processus supracondylaris je kontinuální, jedná se zřejmě o regresivní varietu. Mezipohlavní a stranové rozdíly ve výskytu processus supracondylaris nebyli statisticky významné. Některé variety musculus pronator teres, jako například tendinózní ulnární hlava, mohou predisponovat vznik úžinového syndromu nervus medianus. Musculus pronator teres se vyskytuje u mnoha druhů savců schopných pronace. U většiny má pouze humerální hlavu, ulnární je vytvořena jen u člověka a antropoidních opic. Foramen supracondylare je přítomno pouze u některých druhů savců. Jeho funkce však není

zcela jasná. Musculus pronator teres vykazuje u lidských plodů stejnou úpravu jako u dospělých jedinců. V průběhu ontogenetické vývoje se začíná diferencovat na přelomu šestého a sedmého gestačního týdne. Chrupavčitý processus supracondylaris není v průběhu ontogenetického vývoje vytvořen ani v embryonálním ani ve fetálním období.

Abstract:

Background. The variability of pronator teres can play a role by the origin of the entrapment syndromes of the median nerve. The attention was paid especially to the relationship of the pronator teres muscle and to the supracondylar process of humerus which can serve as a variable origin place of this muscle. It was also observed the arrangement of these structures in human embryos and fetuses and further analogous formations in selected mammal species. Methods. To find out the occurrence of the supracondylar process, it was performed a study of five historical osteological sets and also a radiographic study of a sample of recent Czech population. The variability of the pronator teres muscle was observed in 68 anatomical sections of the upper extremities. In the comparative part, there were dissected the upper extremities of six selected mammal species. The arrangement of the observed structures was studied by classical dissection in three human fetuses and further there was performed microCT scanning in two embryos. Results. In the set of Vedrovice, the supracondylar process occurred with a frequency of 2.1% owing to number of individuals and 1.1% owing to number of bones. In the set of Hoštice, the supracondylar process occurred also with a frequency of 2.1% owing to number of individuals and 1.1% owing to number of bones. In the set of Brno – Maloměřice, the supracondylar process occurred with a frequency of 5.1% owing to number of individuals and 2.6% owing to number of bones. In the set of Pohansko, the supracondylar process occurred with a frequency of 1.8% owing to number of individuals and 0.9% owing to number of bones. In osteological collection, the supracondylar process occurred with a frequency of 2.1% owing to number of bones. The radiological study demonstrated the occurrence of the supracondylar process in 0.6%. In the studied upper extremities of adults, the humeral head of pronator teres starts always from the medial epicondyle of humerus and in 70.6% of cases also from the intermuscular medial brachial septum. The humeral head was double in 1.5% of cases. The ulnar head absent in 4.4% of cases; in 66.2% of cases it was muscular, in 20% it was tendinous and in 13.8% it was mixed. The average height of the muscle beginning from humerus in the area of the medial epicondyle varied about 20 mm. The median nerve passed between both heads of pronator teres in 85.3%, it pierced the ulnar head in 5.9% and passed under both heads in 2.9%. In one case the ulnar vessels passed between the both heads of pronator teres together with the median nerve. The supracondylar process or Struthers´ ligament weren´t found on the studied material. It was performed comparative dissection of six mammal species, four of them (common squirrel monkey, lemur, cat and Djungarian hamster) had created the supracondylar foramen. The pronator teres started in Djungarian hamster and in lemur both from the medial epicondyle and from the bony arch closing off the supracondylar foramen. In cat, golden hamster, common squirrel monkey and dog, it started only from the medial epicondyle. In the common squirrel monkey, the pronator teres began with two heads like in man. In three observed human foetuses, there was found out the same arrangement of the pronator teres (including the relationship to the median nerve) as in man. The presence of the supracondylar process wasn´t recorded. There wasn´t also noticed the presence of the supracondylar process in two human embryos. Conclusions. The occurrence of the supracondylar process is continuous; it goes evidently about the regressive variation. The gender differences and side differences were not statistically significant. Some varietions of the pronator teres, e.g. the tendinous ulnar head, can predispose to the origin of the median nerve entrapment syndrome. The pronator teres occurs in many mammal species which are able to pronation. It has mostly only the humeral head; the ulnar one is formed

only in man and anthropoid apes. The supracondylar foramen is present only in some mammal species. Its function is not entirely clear. The pronator teres manifests the same arrangement in the human fetuses as in adults. It begins to differentiate at the turn of the 6th and 7th week of the ontogenetic development. The cartilaginous supracondylar process is not formed during ontogenetic development, in both embryonic and fetal period.
