Laboratory metods of monitoring anticoagulant therapy

Abstract:

The aim of this work is to compare and evaluate results between recombinant and tissue thromboplastin in determining prothrombin test. Research deals with comparing results of reagents provided that results of reagents with human tissue thromboplastin is not significantly affected by disease lupus anticoagulant and comparing results of reagents in patients with the same anticoagulant therapy. Next aim is to find what are the advantages and disadvantages of the reagents. Collection of materials is focused on patients with anticoagulation therapy and patients with the disease lupus anticoagulant. Research was made from September to December 2013 in hematology laboratory at the Fakultna nemocnica Ostrava. It was collected 133 samples, where 103 were from patients with anticoagulation therapy and 30 were from patients with the disease lupus anticoagulant. For evaluation was used Student's t-test. Statistical tests were evaluated on the significance level 5 %. Results of test show that reagents are not different from each other. Tissue thromboplastin should not be significantly affected by disease lupus anticoagulant, but when I compared the results from both reagents I conclude that both of them are partially affected by lupus anticoagulant disease. Results of reagents in patients with the same anticoagulant therapy are not different. Due to the fact that test´s results of both reagents are not different, the choice of reagents may not be affected by anticoagulant treatment or lupus anticoagulant disease. I recommend focusing selection of reagents for example on the economic aspect.

Abstract:

Cieľom práce je porovnať a zhodnotiť výsledky medzi rekombinantným a tkanivovým tromboplastínom v rámci stanovenia protrombínového testu. Výskum sa zaoberá porovnaním a zhodnotením výsledkov reagencií za predpokladu, že výsledky reagencie s ľudským tkanivovým tromboplastínom nebudú výrazne ovplyvňované ochorením lupus antikoagulans a porovnaním výsledkov reagencií u pacientov s rovnakou antikoagulačnou liečbou. Ďalším cieľom je zistiť, aké sú výhody či nevýhody daných reagencií. Zber materiálov bol zameraný na pacientov na antikoagulačnej liečbe a pacientov s ochorením lupus antikoagulans. Výskum prebiehal v období od septembra do decembra 2013 v hematologickom laboratóriu vo Fakultnej nemocnici Ostrava. Celkom bolo vyzbieraných 133 vzoriek z toho 103 od pacientov na antikoagulačnej liečbe a 30 od pacientov s ochorením lupus antikoagulans. Pre hodnotenie bol použitý Studentov t-test. Štatistické testy boli hodnotené na hladine významnosti 5 %. Z výsledkov testu celkovo vyplynulo, že reagencie sa od seba neodlišujú. Hoci tkanivový tromboplastín nie je výrazne ovplyvnený ochorením lupus antikoagulans, v porovnaní s výsledkami od rekombinantného tromboplastínu som dospela k záveru, že reagencie vydávajú štatisticky neodlišné hodnoty a teda hodnoty oboch reagencií sú čiastočne ovplyvňované týmto ochorením. Výsledky reagencií u pacientov s rovnakou antikoagulačnou liečbou taktiež nie sú odlišné. Vzhľadom na to, že výsledky testov oboch reagencií nie sú odlišné, výber reagencií nemusí byť ovplyvnený liečbou či ochorením lupus antikoagulans. Okrem toho, analytický prístroj odoberá v oboch prípadoch rovnaké množstvo reagencie. Preto odporúčam zamerať výber reagencie napríklad na ekonomickú stránku.
