Detection and analysis of outliers in clinical registries

Abstract:

Srdeční selhání způsobuje v České republice až 50 % všech úmrtí. Pro jeho výzkum byl zřízen klinický registr AHEAD. Cílem práce je detekovat odlehlá pozorování a chyby v tomto registru a pokusit se ho vyčistit od kontaminantů. Pro detekci odlehlých pozorování byla nejprve zvolena jednorozměrná analýza. Pro každý parametr registru byly vytvořeny základní popisné statistiky a v případě parametrů obsahujících reálná čísla také boxploty a q-q-ploty jako testy rozdělení. Kolmogorov-Smirnovův test (KS-test) a t-test byly použity pro odhalení závislosti reálného parametru na některých kategoriálních parametrech. Poslední využitá metoda je PCA analýza, která nám umožňuje zjistit korelaci některých reálných parametrů, objevit pacienty, kteří mají blízké hodnoty parametrů a také odhalit některé odlehlé pacienty, které obě výše popsané metody nenašly. Jako základní software byl použit program Statistica for Windows, dále SPSS Statistics 17.0. a Microsoft Excel. Při jednorozměrné analýze již bylo možné odhalit některé kontaminanty, jako např. abnormální BMI nebo příliš dlouhý pobyt v nemocnici. KS test a t-test odhalily, že při kombinaci určitých parametrů ve vícerozměrném prostoru se zmenšuje rozpětí intervalu hodnot reálných parametrů nastavené pomocí jednorozměrné analýzy. PCA analýza značně snížila množství pacientů, nicméně odhalila některé, kteří by podle fyziologických hodnot a podle zastoupení v souboru dat měli být zdraví. I tito pacienti byli označeni za odlehlé a je třeba je dále sledovat. Registr AHEAD funguje od září roku 2006. Od té doby jím prošlo 2886 pacientů se srdečním selháním. Z dostupné literatury byly zjištěny různé postupy zpracování těchto dat a jednorozměrná i vícerozměrná analýza, která odhalila některá odlehlá pozorování. Dále byly stanoveny podmínky některých parametrů, které budou postupem času zabudovány do definice registru samotného, a tím by se mělo předejít některým kontaminantům v podobě nesprávně zaznamenaných dat.

Abstract:

Acute heart failure causes 50% of all deaths in the Czech Republic. The clinical trial AHEAD was developed for its research. The aim of this work is to detect outlier observations and errors in this trial and to try to clean it from contaminants. For the first detection of outliers the univariate analysis has been chosen. Every parameter of the trial has its basic describing statistics and for real parameters also box-and-whisker plots and quantil-quantil plots as a test of distribution. Multivariate analysis is also used in this work: Kolmogorov-Smirnov test and t-test were used to disclose a dependence of a real parameter on some categorial parameters. PCA analysis helps us to find out a correlation of chosen real parameters and to discover patients with close values of parameters and to detect some outlier patients that both above mentioned methods could not find. As a basic software has been used Statistica for Windows, SPSS Statistic and MS Excel. Univariate analysis found some contaminants like abnormal BMI or too long hospitalization. KS-test and t-test discovered that combination of some parameters in multivariate space lowers the boudaries of real values set up in univariate analysis. PCA analysis decreased the number of patients but it disclosed some of them that should be healthy according to the position on the distribution and the physiological values. These patients were marked as outliers and it is important to take care of them. Register AHEAD exists from September 2006. Since then 2886 patients with heart failure have been recorded. From literature some techniques of data processing and univariate and multivariate analysis for detecting outlier observation have been found out. Some conditions of real parameters have also been fixed and during some time they will be incorporated in the definition of trial itself in order to avoid some contaminants like wrong recorded data.
