Využití podtlakové terapie k uzávěru ran po fasciotomii u kompartment syndromu. Vliv na délku hospitalizace pacienta

Use of vacuum therapy for wound closure after fasciotomy at compartement syndrome. Effect on the length of hospitalization of the patient

Abstract:

Jedná se o retrospektivní komparativní studii, která hodnotí soubory pacientů s kompartment syndromem končetin s/bez použití systému podtlakové terapie. Od roku 2009 jsme ošetřili 79 pacientů s kompartment syndromem končetin. U 39 pacientů (M 33, Ž 6) jsme použili podtlakovou terapii. U 40 pacientů byla léčba s použitím syntetických krytí. V souboru 39 pacientů s použitím NPWT se jednalo ve 32 případech o kompartment syndrom bérce, ve 3 případech o KS stehna a 4 případech o KS předloktí, z toho 1x byl současně kompartment syndrom na paži. V souboru 40 pacientů šlo o léčbu KS bérce ve 35 případech, stehno bylo léčeno 1krát a 4krát předloktí. Z práce vyplýva, že užití podtlakové terapie v léčbě fasciotomických incizí na končetinách výrazně zkracuje dobu hospitalizace; zkracuje se doba od fasciotomie po uzávěr rány. Použití NPWT je jednou z možností krytí fasciotomických ran; její použití nezvyšuje počet komplikací.

Abstract:

This is a retrospective study and assessment of a group of patients suffering from compartment syndrome of extremities, and the use of vacuum therapy system presenting two case studies. Since 2009, we have treated 79 patients with limb compartment syndrome. In 39 patients (M 33, M 6), we used a vacuum therapy. In 40 patients were using synthetic cover. The group of 39 patients using NPWT is involved in 32 cases of lower leg compartment syndrome, 3 cases of KS thighs and 4 cases of CS forearm, of which 1 was simultaneously compartment syndrome in the arm. In a group of 40 patients had treatment CS of tibia in 35 cases, treated thigh was 1 times and 4 times the forearm. Conclusion is, that application of the vacuum therapy in treatment of fasciotomic incisions of extremities significantly shortens the period of hospitalization, shortens the time from closure after fasciotomy wounds. The use of NPWT is one option fasciotomických cover wounds. Its use does not increase the number of complications.
