Abstract:

Úvod: Práce se zabývá fyzioterapií v silových sportech, konkrétně ve strongman sportu. Pomocí fyzioterapeutických konceptů a metod je zajištěna fyzioterapeutická intervence u amatérského silového sportovce - strongmana s degenerativními procesy v oblasti ramenního kloubu, které jsou následkem zranění a přetěžování při trénincích a v závodní sezóně. Cíl práce: Cílem bakalářské práce je přispět k řešení problematiky fyzioterapie u silových sportů, a to volbou vhodných fyzioterapeutických metod, postupů a konceptů u pacienta s tendinózou šlachy dlouhé hlavy bicepsu a kalcifikující tendinitidou m. supraspinatus. Metodika: Hlavní část práce má charakter případové studie, do které byl zařazen jeden pacient - muž ve věku muž 27 let s diagnózou tendinóza šlachy dlouhé hlavy bicepsu a kalcifikující tendinitida m. supraspinatus. Sběr dat probíhal v období od 1. 2. 2021 do 4. 3. 2021, tedy po dobu necelých pěti týdnů. Bylo realizováno celkem deset intervencí s pacientem. Na základě anamnézy a vstupního kineziologického rozboru byl stanoven krátkodobý plán fyzioterapie s využitím následujících konceptů a metod fyzioterapie: Brügger koncept, ACT, BPP, DNS, PIR, PNF, MOB. Po pěti terapiích byl proveden kontrolní kineziologický rozbor. Po skončení desáté terapie byl proveden výstupní kineziologický rozbor a stanoven dlouhodobý plán fyzioterapie. Výsledky: Po deseti terapiích s pacientem došlo k normalizaci tonu měkkých tkání (odstranění patologických bariér), zvýšení svalové síly, zvýšení RP v levém ramenním kloubu do zevní rotace o 20°, odstranění funkčních kloubních blokád, normalizaci svalového napětí (snížení hypertonu a odstranění TrPs), ke snížení svalového zkrácení a také došlo k odstranění patologické pozitivity některých specifických testů na ramenní kloub. Závěr: Fyzioterapeutická intervence v délce deseti terapií měla vliv na zlepšení zdravotního stavu pacienta. Za nejdůležitější považuji zvýšení rozsahu pohybu do zevní rotace, které pro pacienta bylo limitujícím faktorem při tréninku.

Abstract:

Introduction: The thesis deals with physiotherapy in strength sports, specifically in strongman sports. With the help of physiotherapeutic concepts and methods, physiotherapeutic intervention is provided for an amateur strength athlete - strongman with degenerative processes in the area of the shoulder joint, which are the result of injuries and overloading during training and in the racing season. The objective: The aim of the bachelor thesis is to contribute to the solution of physiotherapy in strength sports, by choosing appropriate physiotherapeutic methods, procedures and concepts in patient with tendinosis of the long head of the biceps and calcifying tendinitis of the supraspinatus muscle. Methodology: The main part of the work has the character of a case study, which included one patient - a man, age 27 with a diagnosis tendinosis of the long head of the biceps and calcifying tendinitis of the supraspinatus muscle. Data collection took place in the period from 1. 2. 2021 to 4. 3. 2021, ie for less than five weeks. A total of 10 interventions with the patient were performed. Based on the anamnesis and initial kinesiological analysis, a short-term physiotherapy plan was determined using the following concepts and methods of physiotherapy: Brügger concept, ACT, BPP, DNS, PIR, PNF, MOB. After five therapies, a control kinesiological analysis was performed. At the end of the tenth therapy, a final kinesiological analysis was performed and a long-term physiotherapy plan was determined. Results: After ten therapies with the patient, there was a normalization of soft tissue tone (removal of pathological barriers), increase in muscle strength, increase of ROM in the left shoulder joint to external rotation by 20°, removal of functional joint blockades, normalization of muscle tone (reduction of hypertone and removal of TrPs), reduction of muscle shortening and the pathological positivity of some specific tests for the shoulder joint was also removed. Conclusions: Physiotherapeutic intervention lasting ten therapies had an effect on improving the patient's health. I consider the most important to be the increase in the range of motion to external rotation, which was a limiting factor for the patient during training.
