The evaluation of children´s pain in postoperative period

Abstract:

Aim: The aim of this master thesis is to verify the efficiency of measuring tool CHIPPS (Children and Infants Postoperative Pain Scale) which assesses pain symptoms in children after surgery and that efficiency compare with efficiency of measuring tool FLACC (Face, Legs, Activity, Cry, Consolability scale). Find out, if intensity of the pain is different according to gender and age of children. Methodic: The research was conducted in period between 3rd of December 2012 to 10th of April 2013. We have chosen quantitative investigation. To our research we selected 92 children which meet our criteria. To find out intensity of pain of our respondets in post-operative period we used two measuring instruments CHIPPS and FLACC, an assessment was repeated. For data analysis we used Cronbachs alfa coefficient, Spearman?s correlation coefficient, Wilcoxon's paired test and others. Results: Was found, that instrument CHIPPS is valid and reliable instrument, which is possible to use in conditions of czech hospitals. Internal consistency investigated by Cronbach?s alfa coefficient was 0,783 in first assessment and 0,757 in second assessment. The value of positive correlation CHIPPS was 0,727 (p=0,000) in 1st and also in 2nd assessment. Statistically important difference in pain intensity according to gender was not confirmed. We learned that younger children have bigger pain intensity than elder. The measuring tool FLACC has emerged as a valid and reliable measuring tool, too. Conclusion: Nowadays, we should not underestimate children's pain. The nurse should work with measuring instruments adapted to age and level of cognitive growth of child and to every child she should behave with individual approach. For the assessment of pain intensity in children is also recommended measuring tool CHIPPS, which proved to be reliable.

Cieľ: Cieľom diplomovej práce je overiť využiteľnosť meracieho nástroja CHIPPS (Children and Infants Postoperative Pain Scale), posudzujúceho prejavy bolesti u detí po operácii a efektivitu nástroja porovnať s použitím meracieho nástroja FLACC (Face, Legs, Activity, Cry, Consolability scale). Zistiť, či sa líši intenzita bolesti vzhľadom na pohlavie, vek dieťaťa. Metodika: Výskum prebehol v období od 3. decembra 2012 do 10. apríla 2013. Bolo zvolené kvantitatívne šetrenie. Do výskumnej vzorky bolo zaradených 92 detí, ktoré vyhoveli našim kritériám. U respondentov boli v pooperačnom období použité na zistenie intenzity bolesti dve škály CHIPPS a FLACC, posudzovanie bolo opakované. Na analýzu dát bol použitý Cronbach alfa koeficient, Spearmanov korelačný koeficient, Wilcoxonov párový test a iné. Výsledky: Zistilo sa, že nástroj CHIPPS je valídny a spoľahlivý nástroj, ktorý sa dá využiť aj v podmienkach českých nemocníc. Reliabilita nástroja CHIPPS, zisťovaná prostredníctvom Cronbach alfa bola 0,783 pri 1. posúdení a 0,757 pri 2. posúdení. Hodnota pozitívnej korelácie CHIPPS bola 0,727 (p= 0,000) pri 1. a 2. posúdení. Štatisticky významný rozdiel v intenzite bolesti vzhľadom na pohlavie sa nepotvrdil. Zistilo sa, že väčšiu intenzitu bolesti mali mladšie deti ako staršie. Merací nástroj FLACC sa ukázal tiež ako valídny a spoľahlivý nástroj. Záver: Bolesť u detí netreba ani v dnešnej dobe podceňovať. Sestra by mala pracovať s meracími nástrojmi prispôsobenými veku a stupňu kognitívneho vývinu dieťaťa a ku každému dieťaťu by mala prechovávať individuálny prístup. Na posudzovanie intenzity bolesti u detí odporúčame aj nástroj CHIPPS, ktorý sa ukázal ako spoľahlivý.
