The effect of special training on rotator cuff of handball players

Abstract:

Házená je sportem kontaktním s rychlou intenzitou hry. To s sebou přináší vysoké riziko zranění a bolesti ramene. I Když prevalence úrazů je v házené vysoká, preventivním programům momentálně není věnována přílišná pozornost. Cílem této diplomové práce bylo zjistit vliv speciálního tréninku na rotátorovou manžetu u hráčů házené. Speciální trénink pro házenkáře byl sestaven tak, aby co nejefektivněji ovlivnil rozsah vnitřní rotace v rameni, sílu zevní rotace v rameni, dyskinezi lopatky a mobilitu hrudní páteře. Efekt toho tréninku byl zjišťován pomocí jednotlivých specifických klinických testů na rameno a pomocí porovnání snímků magnetické rezonance před a po tréninku. Snímky magnetické rezonance byly pořízeny pomocí přístroje Siemens o 1,5 Tesla na katedře studií lidského pohybu Pedagogické fakulty Ostravské univerzity. Data z magnetické rezonance byla hodnocena radiologem. Speciální trénink měl střední efekt na rozsah pohybu ramene do flexe (ES = 0,71) a abdukce (ES = 0,84), malý efekt na rozsah pohybu ramene do vnitřní rotace (ES = 0,52) a na mobilitu hrudníku (ES = 0,56). Speciální trénink neměl efekt na kvalitativní ukazatele rotátorové manžety a na rozsah pohybu do extenze a zevní rotace v rameni. Průměrné zvětšení vzdálenosti acromion lopatky a hlavice humeru bylo po tréninku 0,64 ? 0,092 mm.

Abstract:

Handball is a fast paced contact sport that carries a high risk of injury and possible shoulder pain. Despite the occurrence of injuries in handball being very high, not many preventive programs currently receive much attention. The aim of this diploma thesis was to find out the effect of a special training on the rotator cuff of handball players. This special training was build solely for handball players and the aim of it was to affect the extent of internal rotation in the shoulder, the force of external rotation in the shoulder, dyskinesia of the scapula and the mobility of the thoracic spine as effectively as possible. The effectiveness of this training was determined by performing specific individual clinical tests on the shoulder joint and by comparing magnetic resonance images before and afer training. The magnetic resonance images were taken using a Siemens MRI scanner of 1,5 Tesla located at the Department of Human Movement Studies, Faculty of Education in University of Ostrava. All of the collected magnetic resonance data was evaluated by a radiologist. Special training had a moderate effect on the range of arm movement into flexion (ES = 0.71) and abduction (ES = 0.84), a small effect on the range of arm movement into internal rotation (ES = 0.52) and on chest mobility (ES = 0.56). Special training had no effect on the qualitative indicators of the rotator cuff and on the range of motion to extension and external rotation in the shoulder. The average increase in the distance between the acromion of scapula and the humeral head after training was 0.64 ? 0.092 mm.
