Abstract:

Úvod. Fibrilace síní je nejčastěji se vyskytující setrvalá arytmie. Její přítomnost je asociována s vyšší morbiditou a mortalitou nemocných. Chirurgická ablace patří mezi nejefektivnější metody léčby fibrilace síní, zejména její chronické formy. U značné části nemocných umožňuje dosažení stabilního sinusového rytmu a obnovení efektivních síňových kontrakcí. Cílem této prospektivní studie bylo posoudit přítomnost sinusového rytmu a transportní funkce síní po chirurgické ablaci fibrilace síní pomocí kryoenergie a stanovit predikční faktory efektu procedury. Soubor nemocných a metodika. V období od ledna 2005 do září 2006 byla provedena kryoablace levé síně u 100 po sobě jdoucích nemocných s fibrilací síní. Jejich věkový průměr byl 67,6 ± 7,2 roků (rozmezí 50-82 roků), početně mírně převažovali muži nad ženami (59 mužů, 41 žen). Čtyřicet šest pacientů (46 %) mělo před operací paroxyzmální nebo perzistující a 54 (54 %) permanentní fibrilaci síní. U nemocných s permanentní fibrilací síní byla oproti skupině s paroxyzmální a perzistující fibrilací síní přítomna signifikantně větší levá síň (51,4 ± 6,8 mm versus 46,7 ± 5,3 mm) a významnější trikuspidální regurgitace před operací (2,3 ± 0,9 st. versus 1,5 ± 1,1 st. z čtyřstupňové škály). Ablační procedura byla prováděna pomocí flexibilní kryosondy využívající argonový chladící systém (SurgiFrost, CryoCath/Endocare Inc.), vždy jako výkon přidružený k srdeční operaci s primárně jinou indikací. Samostatný výkon na chlopni/chlopních byl realizován u 50 %, kombinace chlopenní chirurgie a revaskularizace myokardu u 34 %, izolovaná chirurgická revaskularizace myokardu u 15 % a jiné zákroky u 1 % nemocných. Sedmdesát čtyři procent pacientů podstoupilo operaci mitrální vady. Klinické a echokardiografické kontroly byly uskutečněny s odstupem 1 (při této kontrole bez provedení echokardiografie), 3,5, 6, 12, 18 a 24 měsíců od operace. Průměrná doba a medián doby sledování byly 22,4, resp. 24 měsíců, rozmezí 15-24 měsíců. Pomocí Kaplanovy-Meierovy analýzy byl stanoven parametr „freedom from atrial fibrillation“ (vycházeli jsme přitom ze srdečního rytmu po 3,5 měsících od operace, zahrnuty byly všechny dokumentované ataky fibrilace síní trvající déle než 30 sekund). Mechanická funkce síní byla hodnocena pomocí echokardiografie (pulzně-dopplerovské vyšetření transmitrálního a transtrikuspidálního průtoku, přítomnost vlny A spektrální dopplerovské křivky; za hemodynamicky účinnou byla kontrakce levé síně považována při vrcholové rychlosti vlny A transmitrálního průtoku ≥ 25 cm/s, efektivní kontrakce pravé síně byla definována jako vrcholová rychlost vlny A transtrikuspidálního průtoku ≥ 10 cm/s). Výsledky. Sinusový rytmus byl dosažen při dimisi a po 1, 3,5, 6, 12, 18 a 24 měsících od operace u 76,4 %, 70,8 %, 79,8 %, 77,4 %, 75,3 %, 80,3 % a 72,5 % nemocných. Po celou dobu sledování byl statisticky významně častější v podskupině paroxyzmální + perzistující fibrilace síní oproti pacientům s permanentní fibrilací síní před operací (p<0,05). „Free from atrial fibrillation“ bylo s odstupem 12, resp. 24 měsíců od operace 68,2 %, resp. 54,4 % pacientů bez ohledu na typ fibrilace síní před operací; 73,9 %, resp. 69,6 % nemocných ve skupině paroxyzmální a perzistující fibrilace síní, a 60,3 %, resp. 35,4 % nemocných ve skupině permanentní fibrilace síní. Rozdíl mezi oběma skupinami byl statisticky významný (p=0,01). Efektivní levosíňové kontrakce (u pacientů se sinusovým rytmem) byly přítomny po 3,5 měsících od operace u 70,2 %, po 6 měsících u 71,2 %, po 12 měsících u 85,3 %, za 18 měsíců u 80,7 % a za 24 měsíců u 89,2 % pacientů. evé síně byla detekována častěji ve skupině paroxyzmální + perzistující ve srovnání se skupinou s původně permanentní fibrilací síní, rozdíl ale nebyl statisticky signifikantní, snad jen s výjimkou kontroly po 3,5 měsících (na hranici statistické významnosti). Transportní funkci pravé síně jsme nalezli po 3,5, 6, 12, 18 a 24 měsících od operace u 97 %, 95,5 %, 98,3 %, 98,2 % a 97,2 % nemocných. Nebyl rozdíl mezi skupinou paroxyzmální + perzistující a permanentní fibrilace síní. Statisticky významnými negativními prediktory dosažení stabilního sinusového rytmu byly: trvání fibrilace síní (p=0,01), narůstající velikost levé síně (při kontrolách za 1, 18 a 24 měsíců, p=0,005, p=0,02 a p=0,01), závažnost mitrální regurgitace (po 1 a 18 měsících, p=0,002, resp. p=0,04) a závažnost trikuspidální regurgitace před operací (po celou dobu sledování, p<0,05). Jako negativní předpovědní faktory obnovení transportní funkce levé síně byly identifikovány: narůstající předoperační velikost levé síně (jen při 3,5měsíční kontrole, p=0,01), závažnost trikuspidální regurgitace před operací (jen při kontrole po 18 a 24 měsících, p=0,006, resp. p=0,004) a před operací přítomná mitrální stenóza (po 3,5 a 6 měsících, p=0,045, resp. p=0,02). Implantace kardiostimulátoru v pooperačním období byla nutná u 5 % (dimise) až 8,5 % pacientů (18 měsíců od operace). Parametr „freedom from stroke“ činil po 1 roce sledování 95,3 % a po 2 letech 93,9 %. Třicetidenní mortalita souboru dosáhla 12 %, celková mortalita 23 %. Závěr. Chirurgická ablace fibrilace síní využívající kryoenergii je efektivní léčebná metoda, umožňující u vysokého procenta nemocných nejen dosažení a udržení sinusového rytmu, ale také časné obnovení efektivní transportní funkce síní. To je spojeno s redukcí zvýšené morbidity, která je asociována s přítomností fibrilace síní. Stanovení predikčních faktorů úspěchu procedury umožňuje selektovat podskupinu nemocných s malým benefitem v porovnání s rizikem operace, jedná se především o pacienty s výrazně dilatovanou levou síní a velmi dlouhou anamnézou permanentní fibrilace síní. Dosažená redukce výskytu ischemických cévních mozkových příhod pomocí chirurgické ablace fibrilace síní byla u nemocných našeho souboru nižší, než je udáváno v literatuře.

Abstract:

Background. Atrial fibrillation is the most frequent sustained arrhythmia. Its presence is associated with higher morbidity and mortality of patients. Surgical ablation is ranked among the most effective techniques for treatment of atrial fibrillation, especially of chronic arrhythmia. Stable sinus rhythm is achieved and effective atrial contractions are restored in the majority of patients. The aim of the prospective study is to assess the presence of sinus rhythm and atrial transport function after surgical ablation of atrial fibrillation using cryoenergy; and to evaluate predictors of the procedural success. Patients and methods. Between January 2005 and September 2006, 100 consecutive patients with atrial fibrillation underwent left atrial cryoablation. Their mean age was 67.6 ± 7.2 years (range, 50 to 82 years). The number of men was slightly higher than the number of women (59 men and 41 women). Forty-six patients (46%) suffered from paroxysmal or persistent atrial fibrillation and 54 (54%) from permanent atrial fibrillation before surgery. Patients with permanent atrial fibrillation had significantly bigger preoperative left atrial diameter (51.4 ± 6.8 mm versus 46.7 ± 5.3 mm) and more severe tricuspid regurgitation (grade 2.3 ± 0.9 versus grade 1.5 ± 1.1, four-level scale) compared to the group with paroxysmal and persistent atrial fibrillation. The ablation procedure was performed with a flexible cryoprobe using an argon-based cooling system (SurgiFrost, CryoCath/Endocare Inc.), always as a concomitant procedure in addition to the cardiac surgery for another indication (primarily). Isolated valve surgery was realized in 50% of patients, combined valve surgery and coronary artery bypass grafting in 34% of patients, isolated coronary artery bypass grafting in 15% and other operations in 1% of patiens. Seventy-four percent of patients underwent mitral valve surgery. Clinical and echocardiographic follow-up included visits at 1 (without echocardiography), 3.5, 6, 12, 18 and 24 months after the operation. The mean and median follow-up were 22.4 and 24 months respectively. Freedom from atrial fibrillation rate was determined using Kaplan-Meier analysis (the heart rhythm at 3.5 months postoperatively was taken as a basis; all documented episodes of atrial fibrillation persisting longer than 30 seconds were included). Atrial mechanical function was assessed by echocardiography (pulsed-wave Doppler examination of the mitral and tricuspid inflow; the presence of the atrial filling – A-wave on the spectral Doppler; peak A-wave velocity of the transmitral flow ≥ 25 cm/s was considered to be hemodynamically effective left atrium contraction; effective contraction of the right atrium was defined as a peak A-wave velocity of the tricuspid flow ≥ 10 cm/s). Results. At discharge and at 1, 3.5, 6, 12, 18 and 24 months after surgery sinus rhythm was achieved in 76.4%, 70.8%, 79.8%, 77.4%, 75.3%, 80.3% and 72.5% of patients respectively. Patients with preoperative paroxysmal and persistent atrial fibrillation, in comparison with patients with permanent atrial fibrillation before surgery, were in sinus rhythm significantly more frequently (p<0.05) all the time of the follow-up. At 12 and 24 months postoperatively, 68.2% and 54.4% of patients (regardless of the preoperative type of atrial fibrillation) were free from atrial fibrillation. Freedom from atrial fibrillation was 73.9% at 12 months and 69.6% at 24 months after surgery in the paroxysmal and persistent atrial fibrillation group, and 60.3% at 12 months and 35.4% at 24 months postoperatively in the permanent atrial fibrillation group. There was significant difference between the two groups (p=0.01). The effective left atrial contractions (in patients with sinus rhythm) were present in 70.2% of patients at 3.5 months, in 71.2% at 6 months, in 85.3% at 12 months, in 80.7% at 18 months and in 89.2% at 24 months after surgery. ected more frequently in the group of patients with paroxysmal and persistent atrial fibrillation compared with the group originally with permanent atrial fibrillation. But the difference was not significant, with the exception of 3.5months control (boundary value). We have found right atrial transport function in 97%, 95.5%, 98.3%, 98.2% and 97.2% of patients at 3.5, 6, 12, 18 and 24 months after operation. There was no significant difference between the group with paroxysmal or persistent atrial fibrillation compared to those with permanent atrial fibrillation. Significant negative predictors for attainment of stable sinus rhythm were: the duration of atrial fibrillation (p=0.01), increasing left atrial size (at 1, 18 and 24 months, p=0.005, p=0.02 and p=0.01 respectively), severity of mitral regurgitation (at 1 and 18 months, p=0.002 and p=0.04 respectively) and severity of tricuspid regurgitation prior to procedure (during all the time of the follow-up, p<0.05). Negative predictors for restoration of the left atrial transport function were: increasing preoperative left atrial size (only at 3.5 months, p=0.01), severity of tricuspid regurgitation before operation (only at 18 and 24 months, p=0.006 and p=0.004 respectively) and preoperative mitral stenosis (at 3.5 and 6 months, p=0.045 and p=0.02 respectively). Five percent (at discharge) to 8.5% of patients (at 18 months) required postoperative permanent pacemaker placement. Freedom from stroke was 95.3% at 1 year and 93.9% at 2 years after surgery. Thirty-day mortality was 12%; the overall mortality was 23%. Conclusions. We conclude, that surgical ablation of atrial fibrillation using cryoenergy is an effective therapeutic method not only for achievement and maintenance of sinus rhythm in relatively high proportion of patients, but also for an early restoration of effective atrial transport function. This method contributes to decreasing the morbidity rates that are associated with the presence of atrial fibrillation. The assessment of the procedural success predictors enables to select a subgroup of patients with low benefit compared with operative risk (patients who have markedly dilated left atrium and very long history of permanent atrial fibrillation). The reduction in occurence of ischemic stroke by surgical ablation of atrial fibrillation was lower in our patients than it was achieved in accordance with literature.
