Active participants or passive patients? Representation of problems in Czech mental health policy and their effects on the subjectification of people with mental illness

Abstract:

Po letech neúspěšných snah připravit reformu a realizovat změny v péči o duševní zdraví ministerstvo zdravotnictví započalo reformní proces a nedávno zveřejnilo dokument Strategie reformy psychiatrické péče (dále Strategie). Předkládaná disertace si klade za cíl tyto současné snahy v oblasti péče o duševní zdraví kriticky zhodnotit. Hlavní předpoklad teoretického zakotvení výzkumu spočívá v tom, že strategické dokumenty a návrhy veřejných politik konstruují problémy a zároveň zahrnují, ať již přímo či nepřímo, řešení těchto problémů. Hlavní výzkumná otázka zní: Jaká témata jsou v České politice péče o duševní zdraví reprezentována jako problémy a jaké dopady mají tyto reprezentace na utváření subjektivity lidí s duševním onemocněním? Zajímá mě, jaké diskursy jsou v rámci dané politiky re/produkovány, jaká je jejich dynamika, základní předpoklady a postupy utváření subjektů. Studie využívá kvalitativní analýzu diskursů, ve které kombinuje různé typy dat, sesbíraných v letech 2012 až 2015. Vychází z veřejně politických dokumentů, rozhovorů, zúčastněných pozorování a dalších souvisejících textů. Analytický rámec výzkumu je založen na přístupu vyvinutém Carol Bacchi „What´s the Problem Represented to be?“ Analýza veřejně politických dokumentů odhalila, že Strategie reprezentuje určitá témata jako problémy, a zároveň opomíjí jiné oblasti. Za účelem pátrání po těchto opomenutích a odlišnostech jsem českou politiku péče o duševní zdraví zasadila do kontextu mezinárodních doporučení (SZO, EU, OSN). Podíváme-li se na dané téma z pohledu analýzy diskursů, to, jak jsou problémy v dané politice reprezentovány, ovlivňuje utváření subjektů (v tomto případě lidí s duševním onemocněním) a vede k re/produkci odlišných diskursů. V průběhu analýzy dokumentů jsem identifikovala čtyři vzájemně se překrývající či odlišující diskursy; avšak česká Strategie je rámována pouze třemi z nich: zákaznickým (consumer), medicínským (medical) a rodičovským (parental) diskursem. Jejich jednotícím prvkem je redukce sociálního postavení jedince a konstruování člověka s duševním onemocněním coby pasivního subjektu. Strategie přehlíží význam prvků čtvrtého diskursu (zmocňujícího; empowering), ačkoli jej mezinárodní dokumenty zdůrazňují. Zmocňující diskurs považuji za důležitý, neboť vyzdvihuje aktivitu a posílení vlastních schopností a poskytuje člověku s duševním onemocněním příležitost rozhodovat se sám za sebe. V další fázi analýzy, která zahrnula i další zdroje dat (rozhovory, zúčastněná pozorování a související texty), jsem se zaměřila na to, jak diskursy fungují v praxi a jak jsou jejich jednotlivé prvky vyjednávány a konstruovány aktéry zapojenými do reformy. Výsledky analýzy poukázaly na dominantní pozici zákaznického a medicínského diskursu. Naopak, zmocňující diskurs představuje alternativní přístup, jak nahlížet politiku a péči o duševní zdraví. Výsledky disertace zesilují obavu, že reforma nepřinese kýžené změny, ale povede k zachování statu quo a přetrvávající převaze institucionální péče a medicínského přístupu k vědění v této oblasti.

Abstract:

After years of unsuccessful attempts to create a reform strategy and change mental health care, the Ministry of Health set about a reform process and recently published the policy text Strategy of Psychiatric Care Reform (hereafter Strategy). The study aims to critically assess the reform process in the field of mental health policy. The leading assumption of the theoretical background of the thesis is that policy proposals construct problems and at the same time contain, explicitly or implicitly, solutions to these problems. The main research question is: What issues are problematized in Czech mental health policy and what subjectification effects are produced by such representations? I am particularly interested in what discourses are re/produced, what their dynamics are, as well as the underlying assumptions and practices of the positioning of subjects. The study employs qualitative discourse analysis of multiple data sources collected between 2012 and 2015, such as policy and strategic documents, participatory observations, interviews with actors involved in the reform, and other related documents. The analytical framework is based on Bacchi’s tool which is called “What’s the problem represented to be?” The analysis of the policy documents revealed that the Strategy represents several issues as problems, but, at the same time, silences others. In order to search for the omissions and differences, I contextualized Czech policy within international (WHO, EU, UN) policies. When we look from the perspective of discourse theory, problematizations lead to different subjectification effects that constitute different discourses. During the analysis of policy documents, I identified four discourses, most of which were overlapping but in some respects competing. However, only three of them frame the Czech Strategy: the consumer, medical, and parental discourses. A common aspect of these three discourses is their reduction of social autonomy and the constitution of people with mental illness as passive subjects. Although the international documents emphasize topics linked to the fourth discourse (i.e. empowering), the Strategy ignores them. I value the empowering discourse as contributing to the activity, opportunity to make choices, and empowerment of people with mental illness. The next stage of the analysis, which involved other data sources, further elaborated the individual discourses and showed how they operate in practice, and how their features are negotiated and constructed by the actors involved in the reform process. The outcomes of the analysis revealed the dominant position of the consumer and medical discourses in mental health policy in the Czech Republic. On the contrary, the empowering discourse represents a reverse model of how to approach policy and care. The dissertation results justify the concern that the reform will not bring the desired changes, but will only lead to maintaining the status quo in Czech mental health care and the continuing domination of institutional care and medical knowledge in this field.
