Selection of patients with normal pressure hydrocephalus for shunt placement

Abstract:

V práci je prezentována problematika normotenzního hydrocefalu se zaměřením na selekci pacientů pacientů k implantaci shuntu. Zpracovaný soubor 82 pacientů je od roku 2003 do roku 2011. Výhodou lumbálního infuzního testu je jeho minimální invazivita a malá časová náročnost, ale nevýhodou menší výtěžnost ve srovnání se zevní lumbální drenáží. Proto u pacientů s negativním nebo hraničním výsledkem lumbálního infuzního testu doporučujeme vyšetřovací protokol doplnit o zevní lumbální drenáž. Programovatelné ventily s gravitačními jednotkami nebo antisifónovým zařízením statisticky významně redukují četnost výskytu komplikací z předrénování. Celkově se tak podílejí na zlepšení výsledků chirurgické terapie pacientů s normotenzním hydrocefalem a redukci nutných revizí zkratového systému. Lepších výsledků dosahují pacienti se sekundárním normotenzním hydrocefalem než pacienti s normotenzním hydrocefalem idiopatickým.

Abstract:

The work presents problems of normal pressure hydrocephalus with a focus on the selection of patients for the implantion of a shunt. The group of 82 patients were analyzed from 2003 to 2011. Lumbar infusion test and external lumbar drainage methods are useful in predicting the effect of shunt implantation, but the disadvantage is lower benefit as compared with external lumbar drainage. Therefore, in patients with negative lumbar infusion test or results close to the thereshold we recommend to complete diagnostic algorithm by external lumbar drainage. Programmable valves with gravitational units or antisiphon devices reduce significantly the incidence of complications from overdrainage. They contribute to improved results of surgical therapy in patients with normal pressure hydrocephalus and reduction of necessary revisions of the shunt. Patients with secondary normal pressure hydrocephalus achieved better results with respect to patients with idiopathic normal pressure hydrocephalus.
