Physiotherapy after fracture of femoral diaphysis

Abstract:

Cíl práce: Cílem bakalářské práce je přispět k řešení dané problematiky pomocí správně zvolených fyzioterapeutických, kinezioterapeutických postupů a konceptů u pacienta po zlomenině diafýzy femuru. Metodika: V této části práce je zpracována kazuistika muže ve věku 21 let s diagnózou zlomeniny diafýzy femuru. Na podkladech vstupního kineziologického rozboru je sestaven krátkodobý fyzioterapeutický plán. Fyzioterapeutický proces trval 13 týdnů v rámci jedenácti návštěv a byl rozdělen na dvě části 4 návštěvy v první části a 7 návštěv ve druhé části. Ve fyzioterapeutické intervenci bylo využito různých konceptů a metod - myofasciální techniky, pasivní pohyb, dechové cvičení, Brügger koncept, prvky Dynamické neuromuskulární facilitace, Proprioreceptivní neuromuskulární facilitace, Senzomotorická stimulace a další. V průběhu čtvrté návštěvy byl proveden kontrolní a doplňující kineziologický rozbor zaměřený především na vyšetření zkrácených struktur, kloubní vůli a pohybové stereotypy. Při posledním setkání je sestaven výstupní kineziologický rozbor společně s vypracováním dlouhodobého fyzioterapeutického plánu. Výsledky: Po fyzioterapeutické intervenci došlo u pacienta k výraznému zlepšení v oblasti rozsahu pohybu v kyčelním a kolenním kloubu, svalové síly v kyčelním i kolenním kloubu a úpravě zkrácených struktur a pohybových stereotypů v oblasti dolní končetiny. Závěr: Vlivem fyzioterapeutické intervence a kvalitní spoluprací s pacientem došlo k zlepšení jeho zdravotního stavu.

Abstract:

The objective: The aim of bachelor's thesis is to contribute to the solution of the given problem by choosing a correct set of physiotherapeutic, kinesitherapy procedures and concepts for a patient following a fracture of femur diaphysis. Method: In this part of the thesis is elaborated on a case history of a man of 21 years with diagnosis of a fracture of femur diaphysis. Based on the materials of the entering kinesiology analysis a short-term physiotherapeutic plan in put into place. Physiotherapeutic process lasted 13 weeks in the form of 11 visits and was devided into two parts: 4 visits in the first part and 7 visits in the second. Within the physiotherapeutic intervention, various concepts and methonds were used - myofascial techniques, passive movement, breathing exercises, Brügger concept, elements of Dynamic neuromuscular facilitation, Proprioreceptive neuromuscular facilitation, Sensomotor stimulation and more. During the fourth visit, a kinesiology analysis was conducted for the purposes of inspection and update, focusing on the analysis of abbreviated structures, joint will and movement stereotypes. The final kinesiology analysis along with the devising of a long-term physiotherapeutic plan is conducted during the last visit. Results: Following the physiotherapeutic process, a major improvement could be seen on the patient's condition in the area of movement range of the hip and knee joints and of muscle strength in hip and knee joints and of adjustment of abbreviated structures and movement stereotypes in the area of the lower limb. Conclusion: The major improvement of the patient's health condition was the result of quality co-operation with the patient and physiotherapeutic intervention.
