Comparison of modern implants in the treatment of distal tibial fracture, experimental analysis and clinical study

Abstract:

Primárním cílem disertační práce je analyzovat skupinu pacientů, léčených pro zlomeninu pilonu tibie, a na základě retrospektivního hodnocení formulovat optimalizovaný operační léčebný algoritmus specifický pro podmínky našeho pracoviště. V teoretické části se práce zaměřuje nejdříve na úvod do problematiky, vysvětluje anatomickou, fyziologickou a patofyziologickou stránku pilonu tibie. Následují kapitoly, které vysvětlují základní princip léčby zlomenin v této oblasti, popisují operační přístupy, způsoby repozice a stabilizace zlomenin. V závěru teoretické části je popsána biomechanická analýza pevnosti a pružnosti jednotlivých typů zevních fixátorů. Samotná klinická práce hodnotí výsledky léčby zlomeniny pilonu tibie u pacientů léčených primárně dlahovou osteosyntézou ve srovnání se skupinou pacientů, kde byl volen fázový způsob ošetření: dočasná repozice a stabilizace pilonu hlezno přemosťujícím zevním fixátorem a druhé době provedenou konverzí na dlahovou osteosyntézu. Výsledky léčby byly srovnávány dle klinických i radiologických známek hojení a AOFAS hodnotícího systému. Do studie byli zařazeni pacienti, kteří byli operováni mezi lety 2013 a 2017 v Úrazové nemocnici v Brně. Stanovené cíle práce: - Srovnání výsledků léčby zlomeniny pilonu primární stabilizací dlahovou osteosyntézou s fázovým způsobem ošetření - Posouzení volby osteosyntetického materiálu s ohledem na kategorizaci zlomeniny dle AO klasifikace. Posouzení pevnosti montáže zevního fixátoru vzhledem k charakteru a umístění tibiálně zavedených pinů. - Návrh a implementace optimální léčebné strategie včetně moderních metod dočasné a finální stabilizace s využitím soudobých druhů implantátů. Součástí práce je také biomechanická analýza, která má za cíl navrhnout optimální formu montáže zevního fixátoru, dočasně stabilizujícího zlomeninu pilonu.

Abstract:

The primary aim of the thesis is to analyze a group of patients treated for tibial pilon fracture and to formulate an optimized surgical treatment algorithm specific for the conditions of our department based on a retrospective evaluation. In the theoretical part, the thesis firstly focuses on the introduction to the problem, explaining the anatomical, physiological and pathophysiological aspects of the tibial pilon. This is followed by chapters that explain the basic principles of treatment of fractures in this area, describe surgical approaches, methods of repositioning and stabilization of fractures. The theoretical part concludes with a biomechanical analysis of the strength and flexibility of different types of external fixators. The clinical work itself evaluates the results of treatment of tibial pilon fractures in patients treated primarily with plate osteosynthesis in comparison with a group of patients where a staged treatment method was chosen: temporary repositioning and stabilization of the tibial pilon with a bridging external fixator and second time conversion to plate osteosynthesis. Treatment outcomes were compared according to clinical and radiological signs of healing and the AOFAS scoring system. Patients who were operated on between 2013 and 2017 at the Trauma Hospital in Brno were included in the study. Stated aims of the study: - To compare the results of the treatment of pilon fracture by primary stabilization with splint osteosynthesis with a staged treatment approach. - Assessment of the choice of osteosynthetic material with regard to the categorization of the fracture according to the AO classification. Assessment of the strength of the external fixator assembly with respect to the nature and location of the tibially inserted pins. - Design and implementation of optimal treatment strategy including modern methods of temporary and final stabilization using contemporary types of implants. The work also includes a biomechanical analysis to propose the optimal form of external fixator fitting, temporarily stabilizing the pilon fracture.
