Abstract:

Mozkové funkce je možné studovat zkoumáním elektrické aktivity během jednoduchých kognitivních úkolů. Jedním z takových úkolů je vizuální oddball paradigma, během něhož jsou vyšetřované osobě opakovaně prezentovány častější neterčové podněty, které ignoruje a vzácnější terčové podněty, na které reaguje stiskem tlačítka a/nebo které počítá. Elektrická aktivita intracerebrálně snímaná během úkolu je zprůměrněna zvlášť pro terčovou a neterčovou variantu za vzniku „event-related potenciálů“ (ERP). Ty jsou složeny z jednotlivých komponent neboli vln, jež zřejmě odráží dílčí mozkové funkce zapojené do úkolu. Výzkum zaměřený na dominantní komponenty registrované během oddball úkolu, kterými jsou intracerebrální vlna P3-like a skalpově snímaná vlna P3, je orientován především na hledání mozkových funkcí, které by k těmto vlnám bylo možné přiřadit. Během vizuálního oddball úkolu jsou registrovány i další komponenty, jež mohou odrážet jiné funkce. Cílem předkládané práce bylo studium P3-like vlny v kontextu celého ERP ve vizuálním oddball úkolu. Účast jednotlivých mozkových oblastí v různých fázích úkolu a jejich zapojení do konkrétních funkcí je možné zkoumat porovnáním intracerebrálně snímaných ERP mezi jednotlivými oblastmi a porovnáním ERP mezi terčovou a neterčovou variantou úkolu. V prezentované studii jsme analyzovali elektrickou aktivitu mozku 14 epileptických pacientů intracerebrálně registrovanou během vizuálního oddball úkolu. Časné, střední i pozdní terčové a neterčové ERP komponenty jsme registrovali ve většině vyšetřených anatomických struktur, což podporuje představu, že do mentálních operací probíhajících během vizuálního oddball úkolu je zapojeno mnoho mozkových struktur. Mezi 102 vyšetřenými oblastmi se konfigurace ERP, hodnocená podle počtu, latence a polarity komponent, lišila jak v terčové tak v neterčové variantě úkolu. P3-like komponenta byla identifikována v terčových ERP extrémně se lišících svou konfigurací. Existence rozdílů mezi oblastmi v konfiguraci ERP dokládá, že mozkové oblasti se do terčové i neterčové varianty úkolu zapojují různým způsobem. Srovnání konfigurace ERP mezi terčovou a neterčovou variantou úkolu ukázalo existenci tří skupin: (1) ERP se shodnou konfigurací v celé délce, (30 oblastí u 11 pacientů), (2) ERP s odlišnou konfigurací v celé délce (31 oblastí u 7 pacientů), (3) ERP s odlišnou konfigurací od určitého okamžiku (41 oblastí u 11 pacientů). Okamžik rozchodu terčového a neterčového ERP ze třetí skupiny nastal v různých fázích stimulus-response intervalu a mohl by tak představovat začátek odlišných funkcí zapojených do úkolu. Všechny tři skupiny ERP byly registrovány v oblastech generujících P3-like vlnu. Tento nález spolu s nálezem generátorů P3-like vlny v oblastech s naprosto odlišnou konfigurací terčového ERP lze považovat za další doklad heterogenity této komponenty, která tak zřejmě představuje elektrofyziologický korelát různých mozkových funkcí. Analýzou časové vazby P3-like vlny na podnět nebo pohyb jsme potvrdili existenci typu vlny časově vázané na pohyb, typu vlny časově vázané na podnět a typu vlny bez časové vazby na pohyb nebo podnět. Vedle obecně předpokládaného významu P3-like vlny v kognitivním zpracování podnětu lze tedy uvažovat i o jejím významu motorickém.

Abstract:

Brain functions can be studied by investigating electrical activity during simple cognitive tasks. One of such tasks is the visual oddball paradigm, during which more frequent nontarget and less frequent target stimuli are repeatedly presented, while the experimental subject reacts only to the target ones by pressing the microswitch button and/or counting the stimuli. Electrical activity is registered intracerebrally during the task and then averaged separately for the target and nontarget responses giving rise to the event-related potentials (ERPs). These potentials are composed of components or waves that probably reflect partial brain functions involved in the task. Intracerebral P3-like and the scalp recorded P3 waves, being the dominant components registered in the oddball task are investigated mainly with the aim to disclose the brain functions that could be associated with these waves. The aim of the presented thesis was to study the P3-like wave in the context of the whole ERP elicited during the visual oddball task. Involvement of the brain regions in different phases of the task and their engagement in particular functions can be studied by comparing intracerebrally recorded ERPs among regions and by comparing the ERPs between the target and nontarget task variants. We analysed electrical brain activity of 14 epileptic patients that was recorded intracerebrally during the visual oddball task. Early, middle, and late target and nontarget ERP components were registered in most examined anatomical structures supporting the view that many brain structures are engaged in the mental operations, which take place in the visual oddball task. Among 102 examined regions the ERP configuration, assessed according to the number, latency, and polarity of the components differed both in the target and nontarget task variants. P3-like component was identified in target ERPs of extremely different configurations. Existing differences in the ERP configuration among regions gives evidence about different engagement of the brain regions during the task. The comparison between the target and nontarget ERP configurations revealed three groups of ERP: (1) ERPs with identical configuration in the whole length, (30 regions in 11 patients), (2) ERPs with different configuration in the whole length, (31 regions in 7 patients), (3) ERPs with different configuration from a certain point of divergence (41 regions in 11 patients). In the latter group the point of divergence of the target and nontarget ERPs came in different phases of the stimulus-response interval and could thus represent onset of different functions engaged in the task. All the three groups of ERP were registered in the regions generating the P3-like wave. This finding together with the finding of P3-like component in regions revealing completely different configuration of the target ERP can be taken as another evidence of the heterogeneity of this component that thus could represent different brain functions. Analysis of the temporal linkage of the P3-like wave to the stimulus and movement proved the existence of the P3-like wave time-locked to the movement, time-locked to the stimulus and time-locked neither to the movement nor to the stimulus. Besides generally accepted relationship of the P3-like waveform to the cognitive stimulus processing one can also speculate about its relationship to the movement.
