Diagnosis and therapy of hyperpronation syndrome in childhood

Abstract:

Z dnešních poznatků je zřejmé, že postavení nohy ovlivní biomechaniku proximální části dolní končeiny a naopak. Proto je důležité znát kinetické řetězce a zaměřit se při terapii hyperpronačního syndromu nejen na oblast chodidla. Cílem této práce bylo posoudit vliv rehabilitační intervence na hyperpronační syndrom u dětí. Se souhlasem zákonných zástupců se do studie zapojilo 20 dětí (věk 5,5 ? 0,4) s oboustrannou valgozitou paty (9,6 ? 2,4). Podle ochoty rodičů spolupracovat na terapii byly děti rozděleny do dvou skupin, kontrolní (bez terapie) a experimentální (s kinezioterapií). Do výzkumu nebyly zařazeny děti s vrozenými vývojovými vadami, traumatologickým poškozením dolních končetin a neurologickým postižením. Individuální terapie probíhala jednou týdně 30 minut po dobu 3 měsíců v biomechanické laboratoři na Ústavu rehabilitace Lékařské fakulty Ostravské univerzity. Kromě individuálních terapií cvičily děti i doma pod dohledem zaškolených rodičů, a to každý den po dobu alespoň 10 minut. Kinezioterapie byla zaměřena na funkční propojení nohy se stabilizátory kyčelního kloubu. Účinek terapie byl objektivizován pomocí goniometru a dynamického chodníku h/p/cosmos?Rehawalk? se softwarem zebris FDM. Měřenými parametry byla statická valgozita paty a během stojné fáze chůze úhlové nastavení nohy, silové zatížení nohy a doba kontaktu s podložkou jednotlivých oddílů nohy. Z výsledků výzkumu je patrné, že již po tříměsíčním pravidelném cvičení lze zlepšit postavení nohy i optimalizovat dynamické parametry chůze. Aktivní terapie může být vhodnou metodou pro léčbu hypepronačního syndromu u dětí.

Abstract:

According to current knowledge, it is clear that the position of the feet will affect the biomechanics of the proximal lower limb and the other way around. Therefore good understanding of kinetic chains is required. Moreover treatment of hyperpronation syndrome shouldn't be focused only on working with the foot. This work is focused on the funtional connection of the foot with hip joint stabilizers. The aim of this diploma thesis was to evaluate the effect of rehabilitaion intervention on hyperpronation syndrome in children. With the consent of the legal representatives, 20 children (age 5.47 ? 0.36) with bilateral heel valgosity (9.6 ? 2.4) participated in the study. According to the willingness of the parents to cooperate in the therapy, the children were divided into two groups, control (without therapy) and experimental (with kinesiotherapy). Children with congenital malformations, traumatological lower limb damage and neurological impairment were not included in the research. Individual therapy took place once a week for 30 minutes for 3 months in the biomechanical laboratory at the Department of Rehabilitation of the Medical Faculty of the University of Ostrava. In addition to individual therapies, children exercised at home under the supervision of pretrained parents every day for at least 10 minutes. The therapy was objectified using a dynamic treadmill h/p/cosmos?Rehawalk? with zebris FDM software and a goniometer. The measured parameters were the static valgosity of the heel, the angular adjustment of the foot during the standing phase of the walk, the force load of the foot and the contact time of the individual sections of the foot during the walk. The results of the research provide statistically significant difference after 3 months of regular exercise in the experimental group. The angle of the heel valgosity decreased and the dynamic parameters of walking were optimized. Active therapy may be a suitable method for the treatment of hypepronation syndrome in children,
