Multi-state models for patient trajectory identification in inpatient care

Abstract:

Tak jako je jedinečný každý pacient, je jedinečná i jeho trajektorie zdravotním systémem. Pro svůj stav je vystaven různým diagnostickým a léčebným výkonům, hospitalizován na různých odděleních, případně v různých nemocnicích. Naší snahou by však mělo být tuto péči systematizovat. Identifikace a popis trajektorií pacientů je prvním krokem k určení slabých míst zdravotního systému. Komplexním nástrojem pro popis trajektorií pacientů jsou tzv. vícestavové modely. Těmi můžeme hodnotit časové intervaly mezi jednotlivými stavy trajektorie pacienta a faktory, které jejich výskyt ovlivňují. V této práci jsou vícestavové modely představeny a využity při tvorbě a analýze typologií trajektorií pacientů s akutním infarktem myokardu a pacientů se zhoubným novotvarem mozku. Identifikovaná léčebná schémata jsou následně diskutována s literaturou. Ani u jednoho z onemocnění nebyla zjištěna významná odchýlení od doporučených postupů a většina z faktorů ovlivňujících průběh léčby měla své opodstatnění doložitelné literaturou.

Abstract:

Just as each patient is unique, so is their trajectory through the healthcare system. Due to their health condition, they are exposed to various diagnostic and therapeutic procedures, hospitalized in various wards or various hospitals. However, our effort should be to systematize this care. Identification and description of patient trajectories is the first step to recognize weaknesses in the healthcare system. The so-called multi-state models are a complex tool for the description of patient trajectories. They allow us to evaluate the time intervals between individual states of the patient's trajectory and the factors that affect their occurrence. In this work, multi-state models are introduced and used in the formation and analysis of typologies of trajectories of patients with acute myocardial infarction and patients with malignant brain tumors. The identified care schemes are then discussed with the literature. No significant deviations from the guidelines were found and most of the factors influencing the course of treatment were justified by the literature.
