Abstract:

Cílem práce je přispět k řešení problematiky volby vhodných fyzioterapeutických postupů, metod a konceptů u pacienta s diagnózou poranění extenzorového aparátu ruky a jeho následky. V této bakalářské práci je zpracována kazuistika devětapadesátileté ženy, které byla provedena tenolýza extenzorů IV. a V. prstu pravé ruky po adhezi šlach následkem předešlého úrazu. Byl proveden vstupní kineziologický rozbor, na jehož základě jsem stanovila krátkodobý fyzioterapeutický plán. Cílem bylo snížení otoku, zvýšení rozsahu pohybu a svalové síly, zlepšení úchopové funkce ruky, ošetření měkkých tkání a zlepšení celkového postavení pravé horní končetiny. Využila jsem různých fyzioterapeutických metod, postupů a konceptů, jako jsou techniky měkkých tkání, aktivní pohyb, míčkování, nácvik úchopů, SMS, ACT, PNF. Celá spolupráce s pacientkou trvala dva měsíce, během kterých proběhlo deset návštěv. Prvních pět v období hospitalizace, následujících pět pak v domácím prostředí. V průběhu celé fyzioterapeutické intervence byla provedena dvě kontrolní kineziologická vyšetření zaměřená na míru otoku, rozsah pohybu a testování úchopů. Z výstupního kineziologického vyšetření vyplývá, že došlo ke zvětšení rozsahu pohybu, snížení otoku a upravení úchopové funkce ruky.

Abstract:

The aim of this thesis is to contribute to the existing solution of choosing the most suitable physiotherapeutic procedures, methods and concepts in a patient diagnosed with a consequence of injury to the extensor apparatus of the hand. This bachelor thesis deals with a case of a 55year-old woman who underwent tenolysis of extensors IV. and V. of a right hand finger after an adhesion of tendons as a result of a past injury. An initial kinesiological analysis was performed on the basis of which I set a short-term physiotherapeutic plan. The aim was to reduce swelling, increase the range of movement and muscle strength, improve the grip function of the hand, treat soft tissue and improve the overall position of the right upper limb. I used various physiotherapeutic methods, procedures and concepts such as soft tissue techniques, active movement, balling, grip training, SMS, ACT, PNF. The cooperation with the patient as a whole lasted two months, during which ten visits took place. The first five in the hospitalization period, the next five in the home environment. Two kinesiological follow-up examinations, focusing on the degree of swelling, range of motion and grip testing, were performed throughout the physiotherapeutic intervention. The outcome kinesiological examination showed that the range of movement was increased, the swelling was reduced and the hand function was adjusted.
