Physiotherapy of one-sided sports loading

Abstract:

Úvod: V této bakalářské práci, jejíž název je fyzioterapie s jednostranným sportovním zatížením, se chci věnovat fyzioterapii ledního hokejisty. V České republice je lední hokej populární sport, avšak ne vždy se dbá o zdraví hráčů. Lední hokej se hraje velmi rychle a kontaktně, a díky tomu dojde k úrazu. Jelikož je hrán jednostranně, dochází zde ke svalovým dysbalancím, které přecházejí do chronického stádia. Dochází zde k bolestem zad, omezenému rozsahu pohybu, skoliotickému držení těla nebo výrazným asymetriím, které negativně ovlivňují zdraví hráčů ať už v brzké či pozdní fázi kariéry. Cíl: Cílem bakalářské práce je přispět k řešení problematiky u jednostranné sportovní zátěže za pomoci vhodně zvolených terapeutických technik, postupů, konceptů nebo metod u pacienta. Metodika: V této bakalářské práci je popsána fyzioterapeutická intervence pacienta, který se věnuje jednostranné sportovní zátěži (lednímu hokeji). U pacienta byl proveden vstupní kineziologický rozbor spolu se specifickými testy. Na základě zjištěných patologií byl navržen krátkodobý fyzioterapeutický plán. Terapie probíhaly 7 týdny, ve kterých jsme zrealizovali 14 terapií. Během terapií byly využity zejména tyto fyzioterapeutický postupy (terapie měkkých tkání asistována nástroji, ischemická kompresní terapie, postizometrická relaxace, tissue flossing, foam rolling, dynamická neuromuskulární stabilizace, akrální koaktivační terapie, posilování s Therabandem nebo také strečink metodou proprioceptivní neuromuskulární facilitace). Poté následoval výstupní kineziologický rozbor spolu s návrhem dlouhodobého fyzioterapeutického plánu. Výsledky: Pomocí fyzioterapeutické intervence došlo k odeznění bolesti spodních zad, zvýšení rozsahu pohybu kyčelních kloubů (zejména do vnitřní rotace), zlepšení dynamiky páteře, zvýšené aktivaci hlubokého stabilizačního systému, úpravě pohybových stereotypů a ke zlepšení držení těla. Závěr: Podařilo se odstranit většinu nalezených patologií, které by mohly pacienta trápit v pozdější fázi kariéry anebo života. Abstract:

Introduction: In this bachelor's thesis, the title of which is physiotherapy with unilateral sports load, I want to focus on the physiotherapy of an ice hockey player. In the Czech Republic, ice hockey is a very popular sport, but the health of the players is not always taken care of. Ice hockey is a very fast and contact sport, in which injuries occur very quickly. Since it is played unilaterally, muscle imbalances occur here, which progress to a chronic stage. There is a scoliotic posture, a limited range of motion or significant asymmetries, which negatively affect the health of players, whether in the early or late stages of their careers. Aim: The aim of the bachelor's thesis is to contribute to solving the problem of unilateral sports load with the help of appropriately chosen therapeutic techniques, procedures, concepts, or methods in the patient. Methodology: In this bachelor's thesis, the physiotherapeutic intervention of a patient who engages in unilateral sports load (ice hockey) is described. The patient underwent an initial kinesiology analysis along with specific tests. Based on the identified pathologies, a short-term physiotherapy plan was designed. Therapies took place over 7 weeks, during which we carried out 14 therapies. During the therapy, the following physiotherapeutic procedures were used (soft tissue therapy assisted by instruments, ischemic compression therapy, postisometric relaxation, tissue flossing, foam rolling, dynamic neuromuscular stabilization, acral coactivation therapy, strengthening with Theraband or stretching using the method of proprioceptive neuromuscular facilitation). This was followed by the final physiotherapy examination and the long-term physiotherapy plan. Results: With the help of physiotherapeutic intervention there was a reduction of lower back pain, increased range of motion of the hip joints (especially internal rotation), improved spine dynamics, increased activation of the deep stabilization system, modification of movement stereotypes and improved body posture. Conclusion: It was possible to remove most of the pathologies found, which could trouble the patient at a later stage of his career or life.
